Overview of the medicine
Rubee 20 mg Tablet contains Omeprazole, a proton pump inhibitor, used to reduce stomach acid production. It is prescribed for conditions like GERD, ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Usually 20 mg once daily for 4-8 weeks, depending on condition. For severe GERD, 40 mg once daily. For H. pylori eradication, 20 mg twice daily for 7-14 days.
Elderly
No dosage adjustment generally needed, but caution with liver impairment.
Renal_impairment
No dosage adjustment required for renal impairment.
How to Take
Take orally, preferably in the morning, at least 30 minutes before a meal. Swallow the tablet whole with water; do not chew or crush.
Mechanism of Action
Omeprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (proton pump) in the gastric parietal cells, thereby inhibiting the final step in gastric acid secretion.
Pharmacokinetics
Onset
Acid inhibition begins within 1 hour, maximal effect within 2 hours.
Excretion
Mainly excreted in urine (about 77%) and feces (about 23%) as metabolites.
Half life
Plasma half-life is about 0.5-1 hour, but duration of acid inhibition is much longer (up to 72 hours) due to irreversible binding.
Absorption
Rapidly absorbed from the gastrointestinal tract, but absorption is variable. Bioavailability is about 30-40% after a single oral dose.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Omeprazole or any component of the formulation
- •Concomitant use with Nelfinavir
Drug Interactions
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of Clopidogrel by inhibiting CYP2C19.
Methotrexate
Increased plasma levels of methotrexate, especially with high doses.
Diazepam, Phenytoin
Increased plasma levels due to inhibition of hepatic metabolism.
Nelfinavir, Atazanavir
Reduced absorption and efficacy of these antiretrovirals.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive. Omeprazole is not dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed. Small amounts excreted in breast milk, but considered generally compatible with breastfeeding after risk/benefit assessment.
