Description
Overview of the medicine
Rupatadine is a second-generation antihistamine used for the symptomatic treatment of seasonal and perennial allergic rhinitis and urticaria in adults and adolescents (aged 12 years and above). It has selective peripheral H1-receptor antagonist activity and also acts as a platelet-activating factor (PAF) antagonist.
Uses & Indications
Dosage
Adults
10 mg once daily, taken with or without food.
Elderly
No dose adjustment is generally required, but caution should be exercised in elderly patients with renal or hepatic impairment due to limited clinical experience.
Renal_impairment
Use with caution in patients with severe renal impairment. Dose adjustment may be necessary in severe cases.

Mechanism of Action
Rupatadine is a long-acting, non-sedating, selective peripheral H1-receptor antagonist. It also has antagonistic activity against platelet-activating factor (PAF) and inhibits the degranulation of mast cells and eosinophils, contributing to its anti-allergic and anti-inflammatory effects.

Onset
Onset of action typically within 1 hour.
Excretion
Approximately 60.9% excreted via feces and 34.6% via urine over 7 days, primarily as metabolites.
Half life
The elimination half-life of rupatadine is approximately 5.9 hours (for parent compound) and approximately 15.9 hours (for active metabolites).
Absorption
Rapidly absorbed after oral administration. Mean Cmax achieved approximately 0.75-1.5 hours after intake.
Metabolism
Extensively metabolized by the liver, primarily via CYP3A4, into several active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to rupatadine or to any of the excipients.
Drug Interactions
CNS depressants (e.g., Alcohol, Benzodiazepines)
May enhance the sedative effects of rupatadine.
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin, Grapefruit juice)
Can increase systemic exposure of rupatadine, potentially leading to increased adverse effects.
