Description
Overview of the medicine
Rymin syrup contains Cyproheptadine Hydrochloride, an antihistamine used to relieve symptoms of various allergic conditions and also functions as an appetite stimulant.
Uses & Indications
Dosage
Adults
Typically, 4 mg (10 ml) three times daily. Maximum recommended daily dose is 32 mg.
Elderly
Start with a lower dose (e.g., 2 mg 2-3 times daily) and adjust based on response and tolerability due to increased sensitivity to side effects.
Renal_impairment
Use with caution. Dosage adjustment may be necessary in severe renal impairment.
How to Take
For oral administration. Can be taken with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Cyproheptadine acts as a potent H1-histamine receptor antagonist, thereby blocking the effects of histamine. It also possesses antiserotonergic and anticholinergic properties.
Pharmacokinetics
Onset
Onset of action is typically within 15-30 minutes.
Excretion
Excreted primarily via the kidneys (urine), with some excretion in feces.
Half life
Elimination half-life ranges from 8 to 16 hours.
Absorption
Well absorbed from the gastrointestinal tract following oral administration.
Metabolism
Extensively metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to cyproheptadine or any component of the formulation
- Acute asthma attack
- Narrow-angle glaucoma
- Symptomatic prostatic hypertrophy
- Bladder neck obstruction
- Stenosing peptic ulcer
- Concomitant use with MAOIs (monoamine oxidase inhibitors)
- Newborn or premature infants
Drug Interactions
MAO Inhibitors
May prolong and intensify the anticholinergic and CNS depressant effects of cyproheptadine.
CNS Depressants (e.g., alcohol, sedatives, hypnotics)
May enhance CNS depressant effects, leading to increased drowsiness or sedation.
Anticholinergics (e.g., tricyclic antidepressants, atropine)
May increase the risk of anticholinergic side effects like dry mouth, urinary retention, and blurred vision.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose include severe drowsiness, dizziness, confusion, dry mouth, dilated pupils, flushed skin, fever, hallucinations, convulsions, and coma. Treatment is symptomatic and supportive, including gastric lavage or activated charcoal. Naloxone may be used to reverse CNS depression in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of fetal harm, but adequate and well-controlled studies in pregnant women are lacking. Use only if clearly needed. Lactation: It is unknown whether cyproheptadine is excreted in human milk. Due to the potential for serious adverse reactions in nursing infants, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
