Description
Overview of the medicine
S-Vom 25 mg tablet contains Prochlorperazine, which is primarily used to treat severe nausea and vomiting. It can also be used for short-term management of anxiety and for certain psychotic disorders, especially when other treatments are not effective.
Uses & Indications
Dosage
Adults
For severe nausea/vomiting: 25 mg once or twice daily. For anxiety: 5-10 mg three to four times daily (25 mg dose might be used for severe cases or specific psychotic indications).
Elderly
Lower doses may be required due to increased sensitivity and potential for adverse effects. Start with 1/2 of adult dose.
Renal_impairment
No specific dose adjustment recommended for mild to moderate impairment; use with caution in severe impairment.
How to Take
Take orally with water, with or without food. Do not crush or chew sustained-release tablets.
Mechanism of Action
Prochlorperazine acts by blocking dopamine D2 receptors in the chemoreceptor trigger zone (CTZ) in the brain, thereby inhibiting signals that cause nausea and vomiting. It also exerts antipsychotic effects by blocking dopamine receptors in the mesolimbic pathway.
Pharmacokinetics
Onset
Oral: 30-40 minutes
Excretion
Excreted primarily in urine and feces as metabolites.
Half life
Variable, typically 6-8 hours, but can be longer due to active metabolites.
Absorption
Readily absorbed from the gastrointestinal tract; significant first-pass metabolism.
Metabolism
Extensively metabolized in the liver by oxidation and glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to prochlorperazine or other phenothiazines
- Severe central nervous system (CNS) depression or coma
- Bone marrow suppression
- Parkinson's disease
- Narrow-angle glaucoma, prostatic hypertrophy
- Children under 2 years of age or weighing less than 9 kg
Drug Interactions
Lithium
Risk of neurotoxicity (e.g., confusion, disorientation, extrapyramidal symptoms).
Antihypertensives
Potentiation of hypotensive effects.
Dopamine agonists (levodopa)
Reduced therapeutic effect of levodopa.
CNS Depressants (alcohol, barbiturates, opioids)
Increased sedative effects and CNS depression.
Anticholinergic drugs (tricyclic antidepressants, atropine)
Potentiated anticholinergic effects (e.g., dry mouth, blurred vision, constipation).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe drowsiness, confusion, disorientation, severe extrapyramidal symptoms (e.g., severe muscle stiffness, tremors), convulsions, severe hypotension, and coma. Treatment is symptomatic and supportive, including maintaining airway, managing hypotension, and administering antiparkinsonian drugs for severe extrapyramidal reactions.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Avoid during late pregnancy due to potential for extrapyramidal symptoms in neonates. Avoid during lactation as prochlorperazine is excreted into breast milk.
