Overview of the medicine
Sacutinil is an Angiotensin II Receptor Blocker (ARB) used in the treatment of hypertension and heart failure.
Uses & Indications
Dosage
Adults
Hypertension: Initial dose 24 mg once daily, adjusted based on blood pressure response, maximum 48 mg once daily. Heart Failure: Initial dose 24 mg once daily, may be doubled at intervals of 2-4 weeks to target dose of 48 mg once daily as tolerated.
Elderly
No initial dose adjustment required for elderly patients (over 65 years); however, monitor renal function and potassium levels. Start with lower dose if concomitant conditions exist.
Renal_impairment
Mild to moderate renal impairment (CrCl 30-60 mL/min): No initial dose adjustment. Severe renal impairment (CrCl < 30 mL/min): Initial dose 12 mg once daily. Close monitoring of renal function and potassium is recommended.
How to Take
Oral administration, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Sacutinil selectively blocks the binding of Angiotensin II to the AT1 receptor, leading to vasodilation, reduced aldosterone secretion, and overall reduction in blood pressure.

Onset
Antihypertensive effect typically observed within 1-2 hours, full effect within 2-4 weeks.
Excretion
Primarily excreted via bile/feces (80-90%), with a smaller portion excreted renally (10-20%).
Half life
Approximately 6-9 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability is approximately 30-40%. Peak plasma concentrations reached within 1-2 hours.
Metabolism
Undergoes minimal hepatic metabolism, primarily excreted as unchanged drug or inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to the active substance or to any of the excipients
- •Pregnancy (second and third trimesters)
- •Concomitant use with aliskiren in patients with diabetes mellitus
- •History of angioedema with previous ACE inhibitor or ARB therapy
- •Severe hepatic impairment
Drug Interactions
Lithium
Increased serum lithium concentrations and lithium toxicity. Monitor serum lithium levels.
CYP450 inhibitors/inducers
Limited interaction due to minimal metabolism via CYP450 enzymes.
Other antihypertensive agents
Additive hypotensive effect. Careful monitoring is advised.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
May attenuate the antihypertensive effect and increase the risk of renal impairment, especially in dehydrated or elderly patients. Monitor renal function.
Potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes
Increased risk of hyperkalemia. Monitor serum potassium levels.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include marked hypotension and dizziness. Management involves symptomatic and supportive measures. If recent ingestion, gastric lavage may be considered. Closely monitor vital signs and electrolytes.
Pregnancy & Lactation
Sacutinil is contraindicated during the second and third trimesters of pregnancy due to risk of fetal injury and death. Discontinue immediately if pregnancy is detected. It is not recommended during breastfeeding due to unknown excretion in human milk.
