Description
Overview of the medicine
Saga 200 mg Tablet contains Etoricoxib, a selective COX-2 inhibitor, used for symptomatic relief in osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and for the treatment of acute pain and primary dysmenorrhea.
Uses & Indications
Dosage
Adults
For acute severe pain or specific conditions as prescribed: 200 mg once daily for a limited duration, strictly under medical supervision. Typically, lower doses (60-120 mg) are used for most indications.
Elderly
No specific dosage adjustment is usually required, but caution is advised due to increased risk of adverse effects.
Renal_impairment
For moderate renal impairment, caution is advised and lower doses may be considered. Contraindicated in severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Take orally once daily, with or without food. It may be taken with food to reduce potential gastrointestinal upset.
Mechanism of Action
Etoricoxib selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, it reduces prostaglandin production, thereby alleviating pain and inflammation.
Pharmacokinetics
Onset
Pain relief within 24 minutes to 2 hours, depending on the condition being treated.
Excretion
Excreted predominantly by the kidneys (approximately 70%) and feces (approximately 20%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations reached in approximately 1 hour.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes (CYP3A4), to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to etoricoxib or NSAIDs
- Active peptic ulceration or gastrointestinal bleeding
- Severe heart failure (NYHA II-IV)
- Severe renal impairment
Drug Interactions
Lithium
Increased plasma levels of lithium, potentially leading to toxicity.
Diuretics
Reduced natriuretic effect and potential for increased renal risk.
Methotrexate
Increased plasma levels of methotrexate, enhancing its toxicity.
Warfarin and other anticoagulants
Increased risk of bleeding; monitor INR closely.
ACE Inhibitors / Angiotensin Receptor Blockers (ARBs)
Reduced antihypertensive effect and increased risk of renal impairment.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal disturbances (nausea, vomiting, abdominal pain), somnolence, and lethargy. There is no specific antidote. Treatment is supportive and symptomatic, including gastric emptying, activated charcoal, and monitoring of vital signs.
Pregnancy & Lactation
Contraindicated during pregnancy, especially in the third trimester due to potential premature closure of the ductus arteriosus and renal dysfunction in the fetus. Not recommended during breastfeeding as it may be excreted in breast milk.

Lab Monitoring
- Liver function tests (LFTs) periodically, especially in long-term use
- Renal function tests (e.g., serum creatinine, BUN) periodically
- Blood pressure monitoring regularly
- Complete blood count (CBC) if long-term use or signs of GI bleeding
Doctor Notes
- Consider lowest effective dose for shortest duration.
- Monitor blood pressure regularly, especially in hypertensive patients.
- Assess cardiovascular and gastrointestinal risk factors before and during treatment.
- Avoid in patients with significant cardiovascular disease or high risk.
Patient Guidelines
- Take the tablet whole with water, do not crush or chew.
- Do not exceed the recommended dose.
- Inform your doctor about all other medications you are taking.
