Description
Overview of the medicine
Salburen-i 100 mcg Inhaler contains Salbutamol, a short-acting beta-2 adrenergic agonist. It is used to relieve symptoms of asthma and chronic obstructive pulmonary disease (COPD) such as wheezing, shortness of breath, and chest tightness. It works by relaxing the muscles in the airways, making it easier to breathe.
Uses & Indications
Dosage
Adults
For symptomatic relief of bronchospasm: 100 or 200 mcg (1 or 2 puffs) as required. For prevention of exercise-induced bronchospasm: 200 mcg (2 puffs) 10-15 minutes before exercise. Max 8 puffs in 24 hours.
Elderly
Similar to adult dosage, but caution is advised in patients with concomitant cardiovascular disease.
Renal_impairment
No specific dosage adjustment is usually required, but caution is advised in severe renal impairment.
How to Take
Inhale orally. Shake the inhaler well before use. Exhale fully, then press the canister and inhale deeply through the mouth. Hold breath for 10 seconds, then exhale slowly. Wait 30 seconds before taking another puff if needed.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors in the bronchial smooth muscle, leading to relaxation of the smooth muscle and bronchodilation. It also inhibits the release of mast cell mediators such as histamine, leukotrienes, and prostaglandin D2, which contribute to bronchoconstriction.
Pharmacokinetics
Onset
Within 5-15 minutes after inhalation
Excretion
Mainly excreted in urine, largely as the inactive sulfate conjugate and unchanged drug.
Half life
Approximately 4-6 hours (systemic)
Absorption
Rapidly absorbed from the lungs, with a small portion absorbed from the gastrointestinal tract after swallowing. Systemic bioavailability is low (around 10-20%).
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate. Small amounts are metabolized in the lungs.
Side Effects
Contraindications
- •Hypersensitivity to Salbutamol or any component of the formulation
- •Should not be used for premature labor (unless for specific, carefully monitored conditions)
Drug Interactions
Digoxin
Increased risk of hypokalemia, which can potentiate digoxin toxicity.
Diuretics
May potentiate hypokalemia, especially at high doses.
Beta-blockers (non-selective)
Can block the bronchodilatory effect of Salbutamol and may cause severe bronchospasm in asthmatic patients.
Tricyclic antidepressants (TCAs) and MAO inhibitors
May potentiate the cardiovascular effects of Salbutamol.
Storage
Store below 30°C. Protect from frost and direct sunlight. Do not freeze. Do not pierce, break or burn the canister, even when empty.
Overdose
Symptoms include tachycardia, tremor, headache, palpitations, and hypokalemia. Management involves symptomatic and supportive treatment. Cardiac monitoring and potassium level checks may be necessary.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit outweighs the potential risk to the fetus. Salbutamol is likely excreted in breast milk; caution is advised during breastfeeding.
