Description
Overview of the medicine
Salbutamol is a short-acting beta2-adrenergic receptor agonist used for the quick relief of bronchospasm in conditions such as asthma and COPD. This syrup formulation is suitable for oral administration.
Uses & Indications
Dosage
Adults
Adults and adolescents over 12 years: 5-10 ml (2-4 mg) three to four times a day.
Elderly
Use with caution; initial doses may be lower.
Renal_impairment
Dosage reduction may be necessary in patients with severe renal impairment. Consult physician for specific recommendations.
How to Take
Administer orally using a measuring spoon or cup. May be taken with or without food.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors primarily located in the bronchial smooth muscle. This stimulation leads to the activation of adenyl cyclase, increased cyclic AMP (cAMP) levels, and subsequent relaxation of bronchial smooth muscle, causing bronchodilation.
Pharmacokinetics
Onset
Bronchodilation typically occurs within 30 minutes after oral administration.
Excretion
Excreted primarily in the urine as unchanged drug and its metabolites.
Half life
Plasma half-life is approximately 4-6 hours.
Absorption
Rapidly and well absorbed after oral administration. Peak plasma concentrations are reached within 2-4 hours.
Metabolism
Extensively metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any component of the formulation.
- Management of threatened abortion.
Drug Interactions
Digoxin
May decrease serum digoxin levels.
Diuretics (e.g., Furosemide)
May exacerbate hypokalemia when co-administered with high doses of beta-agonists.
Beta-blockers (e.g., Propranolol)
Can antagonize the effects of salbutamol, potentially causing severe bronchospasm.
Monoamine Oxidase Inhibitors (MAOIs) and Tricyclic Antidepressants (TCAs)
Increased risk of cardiovascular adverse effects.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, headache, hyperglycemia, hypokalemia, and lactic acidosis. Treatment is supportive and symptomatic, with close monitoring of potassium and glucose levels.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit outweighs the potential risk to the fetus. Salbutamol is excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a physician.

Lab Monitoring
- Serum potassium levels (especially with high doses or concomitant diuretics)
- Blood glucose levels (in diabetic patients or with high doses).
Doctor Notes
- Counsel patients on the difference between rescue (salbutamol) and maintenance medications.
- Monitor for signs of overuse or worsening asthma control, which may indicate a need for stepped-up maintenance therapy.
- Be aware of potential drug interactions, especially with beta-blockers.
Patient Guidelines
- Do not exceed the prescribed dose or frequency.
- Use only for the relief of symptoms (rescue medication).
- If symptoms worsen or do not improve, consult your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double the dose.
Driving Precautions
May cause tremor or dizziness, which could affect the ability to drive or operate machinery. Exercise caution.
Lifestyle Advice
- Avoid smoking and exposure to respiratory irritants.
- Identify and avoid asthma triggers.
- Maintain a healthy lifestyle.
