Description
Overview of the medicine
Salomax-B 90 mcg Inhaler contains Salbutamol (Albuterol), a short-acting beta2-adrenergic agonist, used for the rapid relief of asthma symptoms and prevention of exercise-induced bronchospasm. It works by relaxing the muscles in the airways, making it easier to breathe.
Uses & Indications
Dosage
Adults
For acute bronchospasm: 2 inhalations (180 mcg) repeated every 4-6 hours as needed. For exercise-induced bronchospasm: 2 inhalations 15-30 minutes before exercise.
Elderly
Similar to adult dosage, but may require careful monitoring for cardiovascular side effects.
Renal_impairment
No dose adjustment typically required, as drug is primarily metabolized and rapidly excreted.
How to Take
For oral inhalation only. Shake the inhaler well before each use. Exhale fully, then place the mouthpiece in the mouth, close lips around it, and inhale slowly while pressing down on the canister. Hold breath for 5-10 seconds, then exhale slowly. Wait at least 1 minute between inhalations if taking more than one puff.
Mechanism of Action
Salbutamol selectively stimulates beta2-adrenergic receptors in the bronchi, leading to relaxation of bronchial smooth muscles, bronchodilation, and increased airflow. It has minimal effect on beta1-adrenergic receptors at therapeutic doses.
Pharmacokinetics
Onset
5-15 minutes (inhalation)
Excretion
Mainly excreted in urine as unchanged drug and its metabolite.
Half life
3.7-5 hours (inhalation)
Absorption
Rapidly absorbed from the lungs and gastrointestinal tract (swallowed portion). Peak plasma concentrations occur within 2-4 hours after oral administration, but effect is local and faster via inhalation.
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any component of the formulation.
- Severe milk protein allergy (for some formulations containing lactose).
Drug Interactions
Digoxin
May decrease digoxin levels.
Diuretics (e.g., Furosemide)
May potentiate hypokalemia.
Beta-blockers (e.g., Propranolol)
Can block the bronchodilatory effect of salbutamol and may produce severe bronchospasm in asthmatic patients. Concomitant use is generally contraindicated.
MAO inhibitors or tricyclic antidepressants
Risk of cardiovascular effects (e.g., hypertension, tachycardia) may be increased if used concurrently or within 2 weeks of discontinuation.
Storage
Store at room temperature (20°C to 25°C), away from direct heat or cold. Do not puncture or incinerate. Keep out of reach of children.
Overdose
Symptoms of overdose include tremor, palpitations, tachycardia, headache, and hypokalemia. Management is primarily supportive and symptomatic, focusing on cardiovascular monitoring and correction of electrolyte imbalances.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if salbutamol is excreted in human milk. Use with caution in nursing mothers.

YesClinical Trials
Salbutamol has been extensively studied in numerous clinical trials for its efficacy and safety in treating asthma and COPD over decades. Trials support its use as a rescue medication.
Lab Monitoring
- Monitor serum potassium levels, especially in patients at risk for hypokalemia.
- Monitor lung function (e.g., FEV1, peak flow) to assess therapeutic response.
Doctor Notes
- Emphasize proper inhaler technique to ensure optimal drug delivery.
- Advise patients against increasing dose or frequency without medical consultation.
- Educate patients on distinguishing between rescue (SABA) and maintenance (ICS/LABA) inhalers.
Patient Guidelines
- Read the patient information leaflet carefully before use.
- Always carry your inhaler for quick relief of symptoms.
- Seek medical attention if your symptoms worsen or if you need to use the inhaler more frequently than prescribed.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Generally, salbutamol does not impair the ability to drive or operate machinery. However, if side effects like dizziness or tremor occur, avoid driving or operating heavy machinery.
Lifestyle Advice
- Avoid known asthma triggers (e.g., allergens, irritants).
- Maintain a healthy lifestyle, including regular exercise (if cleared by a doctor) and a balanced diet.
- Quit smoking.
