Description
Overview of the medicine
Salbutamol is a short-acting beta-2 adrenergic receptor agonist used for the relief of bronchospasm in conditions such as asthma and chronic obstructive pulmonary disease (COPD).
Uses & Indications
Dosage
Adults
Initially 2-4 mg (1-2 teaspoons) 3-4 times daily. Maximum 8 mg (4 teaspoons) 4 times daily.
Elderly
Lower doses may be considered, e.g., 2 mg 3-4 times daily, or as directed by physician.
Renal_impairment
No specific dosage adjustment required, but caution is advised in severe renal impairment.
How to Take
Oral administration. Can be taken with or without food. Use a measuring spoon or cup for accurate dosage.
Mechanism of Action
Selectively stimulates beta-2 adrenergic receptors in the smooth muscle of the bronchi, leading to relaxation of the bronchial smooth muscles, bronchodilation, and increased airflow. It also inhibits the release of mast cell mediators.
Pharmacokinetics
Onset
30 minutes (oral syrup)
Excretion
Mainly excreted in the urine, both as unchanged drug and its metabolite.
Half life
Approximately 4-6 hours
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration.
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- •Hypersensitivity to salbutamol or any component of the syrup
- •Threatened abortion during the first or second trimester of pregnancy (though rarely applicable for oral syrup, more for IV forms)
Drug Interactions
Digoxin
Reduced serum digoxin levels have been reported with salbutamol.
Diuretics
Increased risk of hypokalemia.
MAOIs/TCAs
Increased risk of cardiovascular adverse effects.
Beta-blockers
May antagonize the effects of salbutamol, potentially causing severe bronchospasm.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms include tachycardia, tremors, palpitations, hyperglycemia, hypokalemia, and lactic acidosis. Treatment is supportive and symptomatic, including monitoring of potassium and glucose levels.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, use with caution during lactation.
