Overview of the medicine
Saltica 50 mcg Inhalation Capsule contains Salmeterol, a long-acting beta2-adrenergic agonist (LABA) used for the maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It helps to open up the airways in the lungs, making breathing easier.
Uses & Indications
Dosage
Adults
For Asthma/COPD: One 50 mcg capsule inhaled twice daily (morning and evening, approximately 12 hours apart). For EIB prevention: One 50 mcg capsule inhaled at least 30 minutes before exercise, not more than once daily.
Elderly
No dosage adjustment needed.
Renal_impairment
No dosage adjustment needed.
How to Take
For oral inhalation only using a dry powder inhaler device. Do not swallow the capsule. Pierce the capsule with the device and inhale the powder. Rinse mouth with water after use to minimize local side effects, especially if also using an inhaled corticosteroid.
Mechanism of Action
Salmeterol acts locally in the lung as a long-acting beta2-adrenergic agonist. It stimulates beta2-receptors on airway smooth muscle, leading to bronchodilation and relaxation of the airways, thus improving breathing. It does not provide immediate relief for acute bronchospasm.

Onset
Bronchodilation begins within 10-20 minutes, with maximum effect at 3-4 hours.
Excretion
Primarily excreted in feces (approximately 60%) and urine (approximately 25%) as metabolites over 72 hours.
Half life
Approximately 5.5 hours.
Absorption
Systemic absorption is low following inhalation. A small portion is swallowed and absorbed from the gastrointestinal tract, then undergoes extensive first-pass metabolism.
Metabolism
Undergoes extensive first-pass metabolism via hydroxylation, primarily by CYP3A4.
Side Effects
Contraindications
- •Primary treatment of acute episodes of bronchospasm or status asthmaticus.
- •Hypersensitivity to salmeterol or any component of the formulation.
- •Not indicated for patients who can be effectively managed by occasional use of a short-acting beta2-agonist.
Drug Interactions
Beta-blockers
May block the pulmonary effect of salmeterol and produce severe bronchospasm. Avoid concomitant use unless absolutely necessary and with extreme caution.
Other sympathomimetics
Concomitant use with other sympathomimetic agents may potentiate adverse cardiovascular effects.
Tricyclic antidepressants and MAOIs
May potentiate the action of salmeterol on the cardiovascular system. Use with extreme caution.
CYP3A4 inhibitors (e.g., Ketoconazole, Ritonavir)
Strong CYP3A4 inhibitors can significantly increase salmeterol plasma concentrations, leading to increased cardiovascular adverse effects. Concomitant use should be avoided.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep the capsules in their blister pack until use. Keep out of reach of children.
Overdose
Symptoms of overdose may include tachycardia, tremors, palpitations, headache, hypokalemia, and hyperglycemia. Treatment is supportive and symptomatic, with cardiac monitoring recommended in severe cases.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if salmeterol is excreted in human milk; caution is advised during lactation. The decision to continue or discontinue nursing should consider the importance of the drug to the mother.
