Description
Overview of the medicine
Sanicord-71 Topical Solution contains Clobetasol Propionate, a highly potent corticosteroid, used for the short-term treatment of inflammatory and pruritic manifestations of moderate to severe corticosteroid-responsive dermatoses, such as severe psoriasis and recalcitrant eczemas.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin areas once or twice daily. Do not use for more than two consecutive weeks without medical review. Total dosage should not exceed 50g per week.
Elderly
Use with caution. Elderly patients may be more susceptible to systemic absorption and local side effects (e.g., skin thinning). Monitor closely.
Renal_impairment
No specific dose adjustment is generally required for topical application. Monitor for systemic side effects if significant absorption occurs.
How to Take
For external use only. Apply a thin film to the affected skin area. Rub in gently and completely. Do not cover the treated area with occlusive dressings unless specifically directed by a physician. Wash hands thoroughly before and after application.
Mechanism of Action
Clobetasol Propionate, a potent corticosteroid, acts by inducing phospholipase A2 inhibitory proteins, called lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. This leads to anti-inflammatory, antipruritic, and vasoconstrictive effects.
Pharmacokinetics
Onset
Local anti-inflammatory effects typically observed within hours to days of application.
Excretion
Excreted via the kidneys (urine) and bile (feces) once systemically absorbed.
Half life
Systemically absorbed Clobetasol Propionate has a plasma half-life of approximately 3-4 hours.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase significantly with occlusion, extensive application, prolonged use, or compromised skin barrier. Peak plasma levels are generally undetectable.
Metabolism
Primarily metabolized in the liver, similar to other corticosteroids, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any components of the formulation
- Rosacea
- Perioral dermatitis
- Acne vulgaris
- Pruritus without inflammation
- Perianal and genital pruritus
- Untreated cutaneous infections (viral, bacterial, fungal)
- In children under 1 year of age
Drug Interactions
Other topical corticosteroids
Avoid concomitant use to prevent additive systemic absorption and local side effects like skin atrophy.
CYP3A4 inhibitors (e.g., Ritonavir, Itraconazole)
Concurrent administration may increase systemic exposure to Clobetasol, leading to potential systemic corticosteroid effects. Exercise caution and consider dosage reduction or interruption.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children. Do not freeze.
Overdose
Acute overdose with Sanicord-71 Topical Solution is unlikely due to its topical route of administration. However, chronic overdose or misuse (e.g., large areas, prolonged use, under occlusion) can lead to systemic effects such as Cushing's syndrome, hyperglycemia, and HPA axis suppression. Treatment is symptomatic and supportive; gradual withdrawal if HPA axis suppression is confirmed.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether topical administration of corticosteroids could result in sufficient systemic absorption to produce detectable quantities in breast milk. Use with caution in nursing mothers.

Lab Monitoring
- HPA axis function tests (e.g., morning plasma cortisol, ACTH stimulation test) if prolonged or extensive use is anticipated or suspected of systemic absorption.
- Monitor blood glucose in patients susceptible to hyperglycemia, especially with systemic absorption.
Doctor Notes
- Emphasize short-term use (max 2 weeks) and proper application technique to minimize systemic absorption and local side effects.
- Avoid use on sensitive skin areas (face, groin, axillae) without specific indication and strict monitoring.
- Counsel patients on signs of HPA axis suppression and skin atrophy.
Patient Guidelines
- Use only as prescribed by your doctor and for the shortest duration necessary.
- Do not use on the face, groin, or underarms unless specifically instructed by a physician.
- Avoid contact with eyes. If contact occurs, rinse thoroughly with water.
- Do not apply to open wounds or broken skin.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not apply a double dose to make up for a missed one.
Driving Precautions
Sanicord-71 Topical Solution is unlikely to affect the ability to drive or operate machinery, as it is applied topically with minimal systemic absorption.
Lifestyle Advice
- Maintain good skin hygiene and keep affected areas clean and dry.
- Avoid irritants or allergens that may worsen your skin condition.
- Follow up with your doctor regularly to monitor treatment progress and potential side effects.
