Overview of the medicine
Amlodipine is a long-acting calcium channel blocker used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily. Maximum dose is 10 mg once daily. Dose adjustment may be needed based on patient response and tolerability.
Elderly
Initial dose is 2.5 mg once daily. Dose titration should be done carefully.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
Hepatic_impairment
Initial dose is 2.5 mg once daily. Dose should be increased cautiously.
How to Take
Take orally with a glass of water, with or without food, at the same time each day.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This causes peripheral arterial vasodilation, reducing peripheral vascular resistance and blood pressure. It also dilates coronary arteries and arterioles, increasing myocardial oxygen delivery.

Side Effects
Contraindications

Drug Interactions
Sildenafil
Potential for additive hypotensive effects; monitor blood pressure.
Simvastatin
Limit simvastatin dose to 20 mg daily when co-administered with amlodipine.
Clarithromycin
Increased risk of hypotension and acute kidney injury.
CYP3A4 Inducers (e.g., rifampicin, St. John's wort)
May decrease amlodipine plasma concentrations.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
May increase amlodipine exposure, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves symptomatic treatment, including cardiovascular support, close monitoring of blood pressure, and in severe cases, intravenous calcium gluconate or vasopressors may be necessary.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether amlodipine is excreted in human milk; use with caution in lactating women.
