Description
Overview of the medicine
SB-Corac D is a combination medicine containing Valsartan, an Angiotensin II Receptor Blocker (ARB), and Hydrochlorothiazide, a thiazide diuretic. It is primarily used to treat essential hypertension (high blood pressure) when monotherapy is not sufficient.
Uses & Indications
Dosage
Adults
The usual recommended dose is one tablet (Valsartan 80 mg + Hydrochlorothiazide 12.5 mg) once daily. Dosage can be titrated based on blood pressure response after 2-4 weeks.
Elderly
No special initial dose adjustments are required for elderly patients, but renal function should be monitored.
Renal_impairment
Not recommended in patients with severe renal impairment (creatinine clearance <30 mL/min). No dose adjustment needed for mild to moderate renal impairment, but caution is advised.
How to Take
SB-Corac D tablets should be swallowed whole with water, with or without food, preferably at the same time each day.
Mechanism of Action
Valsartan blocks the binding of angiotensin II to its AT1 receptor, leading to vasodilation and decreased blood pressure. Hydrochlorothiazide increases the excretion of sodium and water by the kidneys, reducing blood volume and peripheral resistance, thus lowering blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect generally observed within 2 hours, full effect after 2-4 weeks.
Excretion
Valsartan: primarily fecal (83%), renal (13%). Hydrochlorothiazide: primarily renal (95-100%) as unchanged drug.
Half life
Valsartan: 6-9 hours. Hydrochlorothiazide: 5-15 hours.
Absorption
Valsartan: rapidly absorbed, bioavailability ~25%. Hydrochlorothiazide: absorbed, bioavailability ~65-75%. Peak plasma concentrations within 2-4 hours for Valsartan and 1-2.5 hours for Hydrochlorothiazide.
Metabolism
Valsartan: minimally metabolized (less than 20%) by CYP450. Hydrochlorothiazide: not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to valsartan, hydrochlorothiazide, or any sulfonamide-derived drugs.
- •Anuria.
- •Severe hepatic impairment, biliary cirrhosis, and cholestasis.
- •Severe renal impairment (creatinine clearance <30 mL/min).
- •Pregnancy (second and third trimesters).
- •Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 mL/min/1.73 m²).
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity; monitor lithium levels carefully.
Corticosteroids, ACTH
May intensify electrolyte depletion, particularly hypokalemia.
Other antihypertensive agents
Additive hypotensive effect.
Alcohol, barbiturates, or narcotics
May potentiate orthostatic hypotension.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal dysfunction, especially in elderly or dehydrated patients.
Potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes
May lead to increased serum potassium levels.
Storage
Store in a cool and dry place, away from light. Do not store above 30°C. Keep out of reach of children.
Overdose
Symptoms of overdose are likely to be hypotension, dizziness, and possibly electrolyte disturbances (hypokalemia, hyponatremia). Treatment is symptomatic and supportive. Induce emesis or perform gastric lavage if recent ingestion. Administer activated charcoal. Correct fluid and electrolyte imbalances.
Pregnancy & Lactation
Contraindicated during the second and third trimesters of pregnancy due to potential fetal toxicity. Use during the first trimester is not recommended. Not recommended during breastfeeding as both components are excreted in human milk.
