Description
Overview of the medicine
Gliclazide 80 mg tablet is an oral antidiabetic medicine used to control high blood sugar levels in adults with type 2 diabetes mellitus, especially when diet and exercise alone are not enough. It belongs to the sulfonylurea class of drugs.
Uses & Indications
Dosage
Adults
Initial dose: 40-80 mg once daily, preferably with breakfast. The dose can be increased gradually up to a maximum of 320 mg daily, usually given in two divided doses for doses above 160 mg.
Elderly
Similar to adult dosage, but titration should be done carefully, starting with a lower dose to avoid hypoglycemia.
Renal_impairment
Generally, no dose adjustment is required for mild to moderate renal impairment (creatinine clearance > 30 mL/min). Close monitoring is recommended. Avoid in severe renal impairment.
How to Take
Administer orally, usually once daily with breakfast. For higher doses (above 160 mg), it may be divided into two doses. Swallow the tablet whole with water; do not chew or crush.
Mechanism of Action
Gliclazide works primarily by stimulating insulin secretion from the pancreatic beta cells. It binds to a specific receptor on the beta-cell membrane, leading to the closure of ATP-sensitive potassium channels, membrane depolarization, and opening of calcium channels. The influx of calcium promotes the exocytosis of insulin.

Extensively metabolized in the liver by CYP2C9, primarily to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Gliclazide or other sulfonylureas
- Type 1 diabetes mellitus
- Diabetic ketoacidosis, diabetic pre-coma and coma
- Severe renal or hepatic impairment
- Concomitant treatment with miconazole (systemic route)
Drug Interactions
Miconazole (systemic route)
Potentiates the hypoglycemic effect, potentially leading to severe hypoglycemia.
Phenylbutazone, other NSAIDs, salicylates
May enhance the hypoglycemic effect.
Beta-blockers, ACE inhibitors, fluconazole, H2-receptor antagonists
May increase the hypoglycemic effect. Beta-blockers can also mask symptoms of hypoglycemia.
Corticosteroids, danazol, chlorpromazine, loop diuretics, oral contraceptives
May reduce the hypoglycemic effect, potentially leading to hyperglycemia.

Store in a cool and dry place, below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose of Gliclazide can lead to severe hypoglycemia, which may present with symptoms like sweating, tremor, hunger, confusion, and unconsciousness. Management involves immediate administration of glucose (oral or intravenous) and close monitoring of blood glucose levels for at least 24-48 hours.
Pregnancy & Lactation
Gliclazide is not recommended during pregnancy. Insulin therapy is preferred for controlling diabetes in pregnant women. It is unknown whether Gliclazide is excreted in breast milk, so it should be avoided during lactation.
