Description
Overview of the medicine
SB-Glic XR 60 mg Tablet is an oral antidiabetic medicine used to control high blood sugar levels in adults with type 2 diabetes mellitus. It helps the body produce more insulin and utilize it more effectively.
Uses & Indications
Dosage
Adults
Initial dose is 30 mg once daily, preferably with breakfast. Dose may be increased gradually up to a maximum of 120 mg daily, based on blood glucose response.
Elderly
No specific dose adjustment is required, but initiation with a low dose (30 mg) and careful monitoring are recommended due to increased risk of hypoglycemia.
Renal_impairment
No dose adjustment needed in mild to moderate renal impairment, but careful monitoring is advised. Contraindicated in severe renal impairment.
How to Take
Take orally once daily, preferably with breakfast. Swallow the tablet whole; do not chew, crush, or divide it.
Mechanism of Action
Gliclazide stimulates insulin release from the pancreatic β-cells by binding to sulfonylurea receptors, thereby closing ATP-sensitive potassium channels. This depolarization leads to an influx of calcium ions, triggering insulin secretion. It also has extrapancreatic effects and helps reduce platelet aggregation.
Pharmacokinetics
Onset
Slower onset due to extended-release formulation, but sustained blood glucose lowering effect over 24 hours.
Excretion
Excreted mainly in urine (60-70%) and feces (10-20%) as metabolites.
Half life
Approximately 12-20 hours for the modified-release formulation.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are reached about 6-12 hours after administration of the extended-release formulation. Absorption is prolonged and steady.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to gliclazide or other sulfonylureas
- Type 1 diabetes mellitus
- Diabetic ketoacidosis
- Severe renal or hepatic insufficiency
- Pregnancy and lactation
- Concomitant use with miconazole
Drug Interactions
Alcohol
Can increase the hypoglycemic effect and may lead to disulfiram-like reactions.
Miconazole
Increases the hypoglycemic effect, leading to severe hypoglycemia. Concomitant use is contraindicated.
Beta-blockers (e.g., Propranolol)
Can mask symptoms of hypoglycemia and potentiate the blood sugar-lowering effect.
NSAIDs (e.g., Phenylbutazone, Ibuprofen)
Potentiate the hypoglycemic effect of gliclazide, increasing the risk of hypoglycemia.
ACE inhibitors (e.g., Captopril, Enalapril)
May enhance the blood glucose-lowering effect of gliclazide.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose can lead to severe hypoglycemia. Symptoms include sweating, shakiness, confusion, and unconsciousness. Immediate management involves administering glucose (oral or intravenous) and close monitoring.
Pregnancy & Lactation
Gliclazide is not recommended during pregnancy and breastfeeding. Insulin is the preferred treatment for diabetes in pregnant women. Consult your doctor for appropriate management.
