Description
Overview of the medicine
sb-glim 1 mg Tablet is an oral antidiabetic medicine used to lower blood glucose levels in patients with type 2 diabetes mellitus, primarily as an adjunct to diet and exercise.
Uses & Indications
Dosage
Adults
Initial dose 1 mg once daily, administered with breakfast or the first main meal. May be increased in increments of 1 mg every 1-2 weeks based on blood glucose response, up to a maximum of 8 mg daily.
Elderly
Start with 1 mg once daily; dose adjustments should be gradual and cautious due to increased risk of hypoglycemia.
Renal_impairment
Initial dose of 1 mg once daily, with careful titration and monitoring of blood glucose to avoid hypoglycemia.
How to Take
Take orally once daily with breakfast or the first main meal of the day. Do not crush or chew the tablet; swallow whole with water.
Mechanism of Action
Glimepiride stimulates insulin release from pancreatic beta cells, primarily by closing ATP-sensitive potassium channels on the beta cell membrane, leading to depolarization and calcium influx, which triggers insulin secretion.
Pharmacokinetics
Onset
Approximately 2-3 hours
Excretion
Excreted about 60% in urine and 40% in feces, primarily as metabolites.
Half life
Approximately 5-9 hours
Absorption
Well absorbed orally, with absolute bioavailability of 100%.
Metabolism
Extensively metabolized by CYP2C9 in the liver to two main metabolites, one active and one inactive.
Side Effects
Contraindications
- Hypersensitivity to glimepiride or other sulfonylureas
- Diabetic ketoacidosis
- Type 1 diabetes mellitus
- Severe renal or hepatic impairment
Drug Interactions
Warfarin
May increase or decrease the anticoagulant effect of warfarin.
Beta-blockers, ACE inhibitors, NSAIDs
May enhance the hypoglycemic effect of glimepiride.
Thiazide diuretics, corticosteroids, oral contraceptives
May reduce the hypoglycemic effect of glimepiride.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypoglycemia, which may lead to coma. Management involves immediate glucose administration (oral glucose or IV dextrose), followed by continuous glucose infusion and close monitoring.
Pregnancy & Lactation
Not recommended during pregnancy due to potential fetal harm (category C). Use during lactation is generally discouraged as glimepiride may be excreted in breast milk and cause hypoglycemia in the infant.

Lab Monitoring
- Fasting blood glucose (FBG)
- HbA1c
- Renal function tests (e.g., creatinine)
- Liver function tests (LFTs) periodically
Doctor Notes
- Emphasize patient education on hypoglycemia recognition and management.
- Regularly assess renal and hepatic function.
- Consider combination therapy if monotherapy fails to achieve glycemic targets.
Patient Guidelines
- Take medicine exactly as prescribed.
- Monitor blood glucose regularly.
- Carry a source of sugar (e.g., glucose tablets) to treat hypoglycemia.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If a dose is missed, take it as soon as you remember if it's still the same day and you haven't eaten your next meal. If it's almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Caution is advised, especially at the start of treatment or during dose changes, due to the risk of hypoglycemia affecting ability to drive or operate machinery. Patients should be aware of the symptoms of hypoglycemia.
Lifestyle Advice
- Maintain a balanced diet.
- Engage in regular physical activity.
- Avoid excessive alcohol consumption.
- Regularly consult with your healthcare provider.
