Description
Overview of the medicine
sb-losak-plus 100 mg tablet contains Losartan Potassium, an angiotensin II receptor blocker (ARB), and Hydrochlorothiazide, a thiazide diuretic. It is used to treat high blood pressure (hypertension) by combining the effects of both medications to lower blood pressure more effectively.
Uses & Indications
Dosage
Adults
Usually 1 tablet (Losartan 100 mg/Hydrochlorothiazide 25 mg) once daily. Dosage may be adjusted based on blood pressure response.
Elderly
No initial dosage adjustment is generally required, but close monitoring is recommended.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance <30 mL/min). Lower doses may be considered for moderate impairment.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor, inhibiting vasoconstriction and aldosterone secretion. Hydrochlorothiazide inhibits sodium and chloride reabsorption in the distal convoluted tubule, leading to increased excretion of sodium, chloride, and water, thus lowering blood volume and blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect of Losartan within 6 hours, peak effect within 3-6 weeks. Hydrochlorothiazide onset within 2 hours.
Excretion
Losartan and its active metabolite are excreted via urine and bile. Hydrochlorothiazide is primarily excreted unchanged in the urine.
Half life
Losartan: 2 hours; E-3174 (active metabolite): 6-9 hours. Hydrochlorothiazide: 5.6-14.8 hours.
Absorption
Losartan is well absorbed after oral administration but undergoes significant first-pass metabolism. Hydrochlorothiazide is also well absorbed from the gastrointestinal tract.
Metabolism
Losartan is metabolized to an active carboxylic acid metabolite (E-3174) by CYP2C9 and CYP3A4. Hydrochlorothiazide is not metabolized.
Side Effects
Contraindications
- Anuria (inability to produce urine)
- Hypersensitivity to Losartan, Hydrochlorothiazide, or any sulfonamide-derived drugs
- Severe renal impairment (creatinine clearance <30 mL/min)
- Severe hepatic impairment
- Pregnancy (second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
Lithium
Renal clearance of lithium may be reduced, increasing lithium toxicity.
Corticosteroids
May intensify electrolyte depletion, particularly hypokalemia.
Alcohol, Barbiturates, or Narcotics
May potentiate orthostatic hypotension.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal dysfunction.
Potassium-sparing diuretics & Potassium supplements
May lead to hyperkalemia.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hypotension, tachycardia, bradycardia (less common), and electrolyte depletion (hypokalemia, hypochloremia, hyponatremia) due to Hydrochlorothiazide. Treatment is supportive and symptomatic, involving correction of fluid and electrolyte imbalances. Hemodialysis is not effective for Losartan or its active metabolite, but may be useful for Hydrochlorothiazide.
Pregnancy & Lactation
Pregnancy Category D (second and third trimesters) due to risk of fetal harm. Not recommended during breastfeeding as both Losartan and Hydrochlorothiazide are excreted in breast milk.
