sb-Metsita 50 mg tablet contains Sitagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, used to improve glycemic control in adults with type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
The recommended dose is 100 mg once daily. For 50 mg tablets, it might be used as a dose reduction for renal impairment or specific clinical situations.
Elderly
No dose adjustment required based on age, but renal function should be assessed.
Renal_impairment
For mild renal impairment (CrCl ≥50 mL/min), no dose adjustment. For moderate (CrCl ≥30 to <50 mL/min), 50 mg once daily. For severe (CrCl <30 mL/min) or ESRD requiring dialysis, 25 mg once daily.
How to Take
Take orally once daily, with or without food.
Mechanism of Action
Sitagliptin inhibits the enzyme dipeptidyl peptidase-4 (DPP-4), which is responsible for inactivating incretin hormones (GLP-1 and GIP). By inhibiting DPP-4, Sitagliptin increases the levels of active incretin hormones, leading to increased insulin synthesis and release from pancreatic beta cells and decreased glucagon secretion from pancreatic alpha cells in a glucose-dependent manner.

Within 24 hours (glycemic effect).
Excretion
Primarily excreted unchanged in urine (about 79%) via active tubular secretion.
Half life
Approximately 12.4 hours.
Absorption
Rapidly absorbed, peak plasma concentrations typically reached in 1 to 4 hours.
Metabolism
Minimally metabolized, primarily by CYP3A4, with some contribution from CYP2C8.
Side Effects
Contraindications
- •History of serious hypersensitivity reaction to Sitagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
- •Patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis.
Drug Interactions
Digoxin
Monitor digoxin levels if coadministered, as Sitagliptin can slightly increase digoxin concentrations.
Cyclosporine
No clinically meaningful interaction observed.
Strong CYP3A4/2C8 inhibitors/inducers
Minimal interaction expected due to minimal metabolism.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In the event of an overdose, initiate appropriate supportive treatment as dictated by the patient’s clinical status. Sitagliptin is modestly dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in pregnant women are not adequate. Use only if clearly needed. It is not known whether Sitagliptin is excreted in human milk. Exercise caution when administered to a nursing mother.
