Overview of the medicine
Scabex-25-W Emulsion contains Permethrin, a synthetic pyrethroid, used for the treatment of scabies caused by the mite Sarcoptes scabiei. It is applied topically to the skin.
Uses & Indications
Dosage
Adults
Apply emulsion thoroughly to all skin surfaces from the neck to the soles of the feet. Leave on for 8-14 hours, then wash off. One application is usually sufficient, but a second application may be needed after 7-14 days if mites are still present.
Elderly
Same as adult dosage.
Renal_impairment
No specific dosage adjustment required as systemic absorption is minimal.
How to Take
For external use only. Apply a thin layer to clean, dry skin from the neck down to the toes. Pay special attention to areas between fingers and toes, wrists, armpits, genitalia, and buttocks. Do not apply to open wounds or mucous membranes. Wash off after the recommended time.
Mechanism of Action
Permethrin acts on the neuron membrane by disrupting the sodium channel current that regulates the polarization of the membrane. This leads to delayed repolarization and subsequent paralysis and death of the parasites (mites).

Onset
Within hours of application for antiparasitic effect.
Excretion
Mainly through urine as inactive metabolites.
Half life
Systemically absorbed permethrin has a half-life of approximately 2-4 hours.
Absorption
Approximately 2% of the applied permethrin is absorbed systemically through the skin. It is rapidly metabolized.
Metabolism
Hydrolyzed by esterases to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to permethrin, other pyrethroids, pyrethrins, or any component of the formulation.
- Children under 2 months of age (for Permethrin 5%; consult specific product label for 2.5% suitability).
Drug Interactions
Corticosteroids (topical)
Concurrent use may mask symptoms of scabies, making diagnosis difficult. Use with caution.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause severe systemic effects due to minimal absorption. If excessive amounts are applied, wash off thoroughly with soap and water. Ingesting permethrin can cause nausea, vomiting, abdominal pain, and neurological symptoms; seek immediate medical attention.
