Overview of the medicine
Scabicid 25% w/v Emulsion is an antiparasitic medication used to treat scabies and pediculosis (lice infestations) by topical application. It works by poisoning the nervous system of the mites and lice.
Uses & Indications
Dosage
Adults
For scabies: Apply a thin layer over the entire body from the neck down, avoiding the face and mucous membranes. Reapply after 24 hours. A third application may be needed after another 24 hours for severe cases. Wash off after 24 hours. Repeat the treatment after 7 days if live mites or new lesions appear. For pediculosis: Apply to affected hairy areas and wash off after 12-24 hours.
Elderly
No specific dosage adjustment is generally required, but caution should be exercised on fragile skin.
Renal_impairment
No dosage adjustment is specifically required for topical application, as systemic absorption is minimal.
How to Take
For scabies: Take a warm bath and dry thoroughly before application. Apply the emulsion from the neck down, covering all skin surfaces, especially between fingers and toes, wrists, armpits, and buttocks. Avoid applying to the face and mucous membranes (mouth, eyes, nose, genitals). Leave the emulsion on for 24 hours, then wash off thoroughly. Repeat the application as directed by a healthcare professional, usually for 2-3 consecutive nights. Ensure all clothes, bedding, and towels used by the infested person and close contacts are washed in hot water or dry-cleaned. For pediculosis: Apply to dry hair and scalp, ensuring full coverage. Leave for 12-24 hours, then wash thoroughly with shampoo. Remove nits with a fine-toothed comb.
Mechanism of Action
Benzyl benzoate acts as a neurotoxic poison on mites and lice, causing paralysis and death. It penetrates the exoskeleton of the parasites and disrupts their nervous system functions.
Pharmacokinetics
Onset
Relief from itching and eradication of parasites typically occurs within hours to a few days after initiation of treatment.
Excretion
Metabolites are primarily excreted in the urine. Minimal excretion via other routes.
Half life
Not well-defined for topical application; systemically absorbed benzyl benzoate is rapidly metabolized.
Absorption
Minimal systemic absorption occurs after topical application to intact skin. However, absorption can be increased through abraded or inflamed skin.
Metabolism
Systemically absorbed benzyl benzoate is rapidly hydrolyzed to benzoic acid and benzyl alcohol, which are then conjugated and excreted.

Contraindications

Drug Interactions
Other topical preparations

Storage
Store below 30°C in a cool, dry place. Protect from direct sunlight and freezing. Do not refrigerate. Keep the bottle tightly closed.
Overdose
Topical overdose may cause severe skin irritation, redness, burning, and blistering. If ingested, seek immediate medical attention. Symptoms of systemic toxicity may include central nervous system excitation, convulsions, and respiratory depression.

Pregnancy: Use with caution during pregnancy, especially during the first trimester. Consult a doctor. Lactation: It is unknown whether benzyl benzoate is excreted in breast milk. Use with caution during breastfeeding. If used, ensure minimal exposure to the infant.
