Seclotil 40 mg Capsule contains Esomeprazole, a proton pump inhibitor, used to reduce stomach acid production. It is prescribed for conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 40 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted based on patient response.
Elderly
No dose adjustment required for elderly patients based on age alone.
Renal_impairment
No dose adjustment required for patients with renal impairment.
How to Take
Seclotil 40 mg capsule should be swallowed whole with water, at least one hour before a meal. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells, thereby inhibiting the final step of acid secretion. This leads to a dose-dependent inhibition of both basal and stimulated gastric acid secretion.

Onset
Acid secretion inhibition begins within 1 hour.
Excretion
Metabolites are primarily excreted in the urine (about 80%) and feces (about 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; absolute bioavailability is approximately 64% after a single 40 mg dose, increasing to 89% after repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Esomeprazole, substituted benzimidazoles, or any component of the formulation
- Concomitant use with nelfinavir (an antiretroviral drug)
Drug Interactions
Warfarin
Increased INR and prothrombin time have been reported, requiring monitoring.
Clopidogrel
Potential reduction in antiplatelet activity of clopidogrel due to CYP2C19 inhibition, although clinical significance is debated.
Methotrexate
Possible increase in methotrexate levels, especially with high doses.
Nelfinavir, Atazanavir
Esomeprazole decreases plasma concentrations of these antiretrovirals, reducing their efficacy. Concomitant use is contraindicated with nelfinavir and not recommended with atazanavir.
Diazepam, Citalopram, Imipramine, Phenytoin
Esomeprazole may increase plasma levels of drugs metabolized by CYP2C19.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include gastrointestinal disturbances and weakness. Treatment is symptomatic and supportive. Esomeprazole is extensively protein bound and is therefore not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is unknown if esomeprazole is excreted in human milk, use with caution during lactation.
