Overview of the medicine
Seemacheph 125 mg Suspension is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections in children, including respiratory tract infections, urinary tract infections, otitis media, and pharyngitis.
Uses & Indications
Dosage
Adults
Adults: 400 mg once daily or 200 mg every 12 hours, for 7-14 days depending on the infection. (Suspension generally used for children).
Elderly
Elderly patients with normal renal function can receive the usual adult dose. Dose adjustment needed for renal impairment.
Renal_impairment
For creatinine clearance 21-60 ml/min: 300 mg daily. For creatinine clearance < 20 ml/min: 200 mg daily.
How to Take
Administer orally with or without food. Shake the suspension well before each use. Use a calibrated measuring spoon or cup for accurate dosing.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. Cell lysis is then mediated by bacterial cell wall autolytic enzymes.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. Some excretion also occurs via bile.
Half life
Plasma half-life averages 3 to 4 hours.
Absorption
Approximately 40-50% orally absorbed. Absorption is not significantly altered by the presence of food.
Metabolism
Not significantly metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin.
Drug Interactions
Nifedipine
May increase cefixime peak plasma concentration and AUC.
Carbamazepine
Elevated carbamazepine levels may occur, requiring monitoring.
Oral Anticoagulants (e.g., Warfarin)
Increased prothrombin time and INR, potentially leading to increased bleeding risk.
Storage
Store the dry powder below 30°C in a dry place, away from light and moisture. Keep the reconstituted suspension refrigerated (2-8°C) and discard after 7-14 days.
Overdose
In case of overdose, gastric lavage may be indicated. There is no specific antidote. Treatment should be symptomatic and supportive. Hemodialysis or peritoneal dialysis do not remove cefixime from the circulation to a significant extent.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human milk; caution should be exercised when administered to a nursing woman.
