Description
Overview of the medicine
Seematrin 200 mg Suspension is a macrolide antibiotic used to treat various bacterial infections in children.
Uses & Indications
Dosage
Adults
Not typically used for adults at 200 mg suspension strength. Adult doses usually start from 500 mg.
Elderly
No specific dose adjustment is required unless severe renal or hepatic impairment.
Children
For children 6 months and older: 10 mg/kg once daily for 3 days, OR 10 mg/kg on Day 1 followed by 5 mg/kg on Days 2-5.
Renal_impairment
Mild to moderate renal impairment does not require dose adjustment. Use with caution in severe renal impairment (CrCl < 10 ml/min).
Hepatic_impairment
Use with caution in patients with hepatic impairment, as azithromycin is primarily cleared by the liver.
How to Take
Orally, once daily, with or without food. Shake the bottle well before each use. Use the provided measuring spoon or cup for accurate dosing. Finish the full prescribed course.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby interfering with the translation of mRNA.
Pharmacokinetics
Onset
Peak plasma concentration typically reached within 2-3 hours.
Excretion
Primarily excreted unchanged into bile; minor renal excretion (about 6% of the dose).
Half life
Terminal elimination half-life is approximately 2-4 days due to extensive tissue uptake and slow release.
Absorption
Well absorbed orally, bioavailability is approximately 37%. Food may slightly reduce absorption.
Metabolism
Partially metabolized in the liver, primarily by N-demethylation; however, a significant portion is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum digoxin levels. Monitor digoxin levels if co-administered.
Warfarin
Increased anticoagulant effect reported. Monitor INR closely during co-administration.
Nelfinavir
Increased azithromycin concentration. Monitor for known azithromycin side effects.
Antacids (aluminum and magnesium-containing)
May reduce the peak plasma concentration of azithromycin. Administer azithromycin at least 1 hour before or 2 hours after antacids.
QT-prolonging drugs (e.g., amiodarone, sotalol, quinidine)
Increased risk of QT prolongation and Torsades de Pointes. Avoid concomitant use if possible.
Storage
Store the powder below 30°C, protected from light and moisture. Store the reconstituted suspension below 30°C and use within 5 days; do not refrigerate. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Gastric lavage and general supportive measures are recommended.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human breast milk; use with caution in nursing mothers.
