Description
Overview of the medicine
Sefox (Cefixime) is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including those of the ear, nose, throat, lower respiratory tract, urinary tract, and uncomplicated gonorrhea.
Uses & Indications
Dosage
Adults
Adults and children over 10 years or weighing over 50 kg: 200 mg once daily or 100 mg twice daily. For severe infections or gonorrhea, dose may be increased to 400 mg once daily.
Elderly
No special dosage adjustment is required unless renal function is severely impaired.
Renal_impairment
Dosage adjustment is recommended for patients with creatinine clearance < 60 mL/min.
How to Take
Sefox capsules can be taken with or without food. To ensure complete absorption and minimize potential gastrointestinal upset, it is often recommended to take it with food. For oral suspension, shake well before use.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to cell lysis and bacterial death.

Onset
Onset of action within a few hours of administration.
Excretion
Primarily excreted unchanged in the urine (about 50-55% of the absorbed dose) and some via bile/feces.
Half life
Plasma half-life averages 3 to 4 hours.
Absorption
Approximately 40-50% of an oral dose is absorbed, with peak plasma concentrations occurring between 2 to 6 hours.
Metabolism
Cefixime is not extensively metabolized; about 10% may be metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Carbamazepine
Elevated carbamazepine levels have been reported with concurrent cefixime administration.
Aminoglycosides
Potential for additive nephrotoxicity, especially in patients with impaired renal function.
Anticoagulants (e.g., Warfarin)
May increase prothrombin time (INR) and risk of bleeding.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. No specific antidote exists. Supportive therapy is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Should be used during pregnancy only if clearly needed. It is not known whether cefixime is excreted in human milk. Caution should be exercised when administered to a nursing mother.
