Description
Overview of the medicine
Sefrate 125 mg Pediatric Drop is an antibiotic used to treat various bacterial infections in children, including ear infections, throat infections, urinary tract infections, and bronchitis. It belongs to a group of medicines called cephalosporin antibiotics.
Uses & Indications
Dosage
Elderly
Not directly applicable for pediatric drop, but for other formulations, dosage adjustment may be required based on renal function.
Children
For children 6 months to 10 years, the usual dose is 8 mg/kg/day administered as a single dose or in two divided doses every 12 hours. Dosage should not exceed 400 mg/day. Or as directed by the physician.
Renal_impairment
In severe renal impairment (creatinine clearance <30 mL/min), the dosage should be reduced.
How to Take
Administer orally with or without food. Shake the bottle well before each use. Use the provided calibrated dropper for accurate dosing. Complete the full course of treatment even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
1-3 hours
Excretion
Primarily excreted unchanged in the urine (approximately 50-60%) and via biliary/fecal excretion.
Half life
3-4 hours (can be prolonged in renal impairment)
Absorption
Cefixime is approximately 40-50% absorbed orally, with food having a minimal effect on absorption. Peak plasma concentrations are achieved 2-6 hours after oral administration.
Metabolism
Cefixime is not extensively metabolized in the body. No active metabolites have been identified.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of serious hypersensitivity reactions to penicillin.
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
Concomitant use may increase carbamazepine plasma concentrations.
Warfarin and other anticoagulants
May increase prothrombin time (PT) with or without bleeding, so monitor INR.
Storage
Store the dry powder below 30°C in a cool, dry place, protected from light and moisture. Keep out of reach of children. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard any unused portion after the specified shelf life.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
