Overview of the medicine
Cefuroxime axetil is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 500 mg twice daily for most infections. Dosage may vary based on infection severity and type.
Elderly
Similar to adults, but dosage adjustment may be needed in case of significant renal impairment.
Renal_impairment
Dosage adjustment is required. For creatinine clearance 10-29 mL/min, 250 mg once daily. For <10 mL/min, 250 mg every 48 hours.
How to Take
Take with food to enhance absorption and minimize gastrointestinal upset. Swallow the capsule whole with water; do not chew or crush.
Mechanism of Action
It works by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death.

Peak plasma concentrations are typically reached within 2-3 hours after oral administration.
Excretion
Primarily excreted unchanged by glomerular filtration and tubular secretion in the urine.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration and hydrolyzed to active cefuroxime. Absorption is enhanced when taken with food.
Metabolism
Hydrolyzed in the intestinal mucosa and blood to release active cefuroxime. It is not significantly metabolized in the liver.
Side Effects
Contraindications
- Known hypersensitivity to cefuroxime, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity reactions to any other type of beta-lactam antibacterial agents (e.g., penicillins).
Drug Interactions
Probenecid
Concomitant use with probenecid increases cefuroxime plasma concentrations and prolongs its half-life.
Oral Anticoagulants
May potentiate the anticoagulant effect. Monitor INR/PT.
Oral Contraceptives
May reduce the efficacy of oral contraceptives. Advise using additional non-hormonal contraception.
Antacids/H2 Blockers
Drugs that reduce gastric acidity may decrease the absorption of cefuroxime axetil. Take at least 2 hours apart.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include cerebral irritation leading to convulsions. Management is supportive. Hemodialysis can reduce cefuroxime serum levels.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Cefuroxime is excreted in small amounts in breast milk; generally considered safe, but use with caution and monitor the infant for diarrhea or candidiasis.
