Description
Overview of the medicine
Semaxen 7 mg tablet contains Semaglutide, an oral glucagon-like peptide-1 (GLP-1) receptor agonist used for the management of type 2 diabetes mellitus to improve glycemic control. It helps the body release more insulin when blood sugar is high and reduces the amount of sugar made by the liver. It may also be used to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes mellitus and established cardiovascular disease.
Uses & Indications
Dosage
Adults
Initial dose is 3 mg orally once daily for 30 days. After 30 days, increase to 7 mg orally once daily. If additional glycemic control is needed after at least 30 days on 7 mg, the dose can be increased to 14 mg orally once daily.
Elderly
No dose adjustment is required based on age.
Renal_impairment
No dose adjustment is required for patients with mild, moderate, or severe renal impairment, or end-stage renal disease (ESRD). Use with caution in patients with ESRD receiving dialysis due to limited clinical experience.
How to Take
Take the tablet at least 30 minutes before the first food, beverage, or other oral medications of the day with a sip of plain water (no more than 4 ounces or 120 ml). Do not split, crush, or chew the tablet.
Mechanism of Action
Semaglutide is a GLP-1 receptor agonist that mimics the effects of endogenous GLP-1. It enhances glucose-dependent insulin secretion from pancreatic beta cells, suppresses glucagon secretion, slows gastric emptying, and increases satiety. These actions collectively contribute to reduced postprandial and fasting glucose levels.
Pharmacokinetics
Onset
Glycemic effects typically begin within a few weeks with stable daily dosing.
Excretion
Approximately 2/3 is excreted in urine and 1/3 in feces, mainly as metabolites.
Half life
Approximately 1 week (about 165-184 hours), allowing for once-daily dosing.
Absorption
Oral bioavailability is low (~1%) due to degradation in the stomach, but it is enhanced by a co-formulated absorption enhancer (salcaprozate sodium - SNAC). Absorption is sensitive to food, water, and other oral medications.
Metabolism
Primarily metabolized by proteolytic cleavage, followed by fatty acid side-chain modifications. No specific CYP enzyme involvement.
Side Effects
Contraindications
- •Personal or family history of medullary thyroid carcinoma (MTC).
- •Patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
- •Known hypersensitivity to semaglutide or any of the excipients.
Drug Interactions
Oral Medications
Semaglutide delays gastric emptying, which may affect the absorption of concomitantly administered oral medications. Monitor patients taking narrow therapeutic index drugs (e.g., warfarin, levothyroxine) when starting or escalating Semaglutide, and adjust dosage if necessary.
Insulin and Sulfonylureas
Increased risk of hypoglycemia when co-administered with insulin or sulfonylureas. A dose reduction of insulin or sulfonylureas may be required.
Storage
Store below 30°C. Protect from moisture and light. Keep in the original blister pack to protect from moisture. Do not freeze.
Overdose
In the event of an overdose, a prolonged period of hypoglycemia may occur. Appropriate supportive treatment should be initiated according to the patient’s clinical signs and symptoms. Monitoring for hypoglycemia is necessary.
Pregnancy & Lactation
Pregnancy Category C. Limited data on semaglutide use in pregnant women are insufficient to determine a drug-associated risk of adverse developmental outcomes. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether semaglutide is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
