Description
Overview of the medicine
Aripiprazole is an atypical antipsychotic used to treat schizophrenia, bipolar disorder, and as an adjunctive treatment for major depressive disorder. It helps to restore the balance of certain natural substances in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10-15 mg once daily; maintenance 10-30 mg/day. Bipolar mania: Initially 15 mg once daily (monotherapy) or 10-15 mg once daily (adjunctive); maintenance 10-30 mg/day. Major Depressive Disorder (adjunctive): 2-15 mg once daily.
Elderly
Lower starting doses and slower titration recommended, especially in patients with dementia-related psychosis due to increased mortality risk.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take orally once daily with or without food. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Aripiprazole is thought to work through a combination of partial agonism at dopamine D2 and serotonin 5-HT1A receptors, and antagonism at serotonin 5-HT2A receptors. This unique mechanism helps to stabilize dopaminergic and serotonergic systems.
Pharmacokinetics
Onset
Several days to weeks for full therapeutic effect, although some improvements may be seen earlier.
Excretion
Primarily through feces (approx. 55%) and urine (approx. 25%).
Half life
Approximately 75 hours for aripiprazole and 94 hours for its active metabolite dehydro-aripiprazole.
Absorption
Well absorbed orally, peak plasma concentration reached in 3-5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP3A4 enzymes.
Side Effects
Contraindications
- Known hypersensitivity to Aripiprazole or any component of the formulation.
- Dementia-related psychosis in elderly patients (due to increased mortality risk, a Black Box Warning).
Drug Interactions
Antihypertensives
Increased risk of orthostatic hypotension.
Drugs that prolong QT interval
Use with caution.
CYP3A4 inducers (e.g., Carbamazepine, Rifampin)
May decrease Aripiprazole plasma levels; dose increase of Aripiprazole may be needed.
CNS depressants (e.g., Alcohol, Benzodiazepines)
Increased risk of sedation and other CNS depressant effects.
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Itraconazole)
May increase Aripiprazole plasma levels; dose reduction of Aripiprazole by half may be needed.
Strong CYP2D6 inhibitors (e.g., Quinidine, Fluoxetine, Paroxetine)
May increase Aripiprazole plasma levels; dose reduction of Aripiprazole by half may be needed.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tremor, and tachycardia. Severe cases may lead to coma or death. Management is supportive, including gastric lavage, activated charcoal, and monitoring of vital signs and cardiac rhythm. There is no specific antidote.
Pregnancy & Lactation
Pregnancy: Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Neonates exposed to antipsychotics during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Lactation: Not recommended during breastfeeding due to potential excretion in breast milk and possible adverse effects on the infant.

Clinical Trials
Aripiprazole has undergone extensive clinical trials supporting its efficacy and safety in various indications. Ongoing post-marketing surveillance and trials continue to explore long-term effects and new therapeutic uses.
Lab Monitoring
- Regular monitoring of glucose and lipid profiles (fasting plasma glucose, triglycerides, LDL-C, HDL-C) due to risk of metabolic changes.
- Monitor body weight regularly.
- Complete blood count (CBC) if signs of infection develop (especially for severe neutropenia/agranulocytosis).
Doctor Notes
- Monitor patients closely for metabolic changes (weight, glucose, lipids) and extrapyramidal symptoms, especially during initiation and dose adjustments.
- Counsel patients and caregivers about the Black Box Warnings, particularly concerning suicidal ideation in younger patients and increased mortality in elderly dementia patients.
- Advise patients about the risk of orthostatic hypotension and sedation, and instruct them on appropriate precautions.
- Assess for presence of impulse control disorders (e.g., pathological gambling, compulsive eating) during treatment.
Patient Guidelines
- Do not stop taking this medicine suddenly without consulting your doctor, as this may lead to withdrawal symptoms or worsening of your condition.
- Report any unusual or uncontrollable movements, changes in mood or behavior, or suicidal thoughts to your doctor immediately.
- Avoid alcohol while taking this medicine, as it can worsen side effects like drowsiness and dizziness.
- Rise slowly from a sitting or lying position to prevent dizziness due to orthostatic hypotension.
Missed Dose Advice
If a dose is missed, take it as soon as remembered unless it's almost time for the next scheduled dose. In that case, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Sensipin may cause drowsiness, dizziness, or blurred vision, especially at the start of treatment. Avoid driving or operating heavy machinery until you know how this medicine affects you.
Lifestyle Advice
- Maintain a healthy diet and regular exercise to help manage potential weight gain associated with this medication.
- Stay hydrated, especially in hot weather or during physical activity, as antipsychotics can impair body temperature regulation.
- Practice good sleep hygiene to help manage insomnia, a common side effect.
