Description
Overview of the medicine
Sentix (Zolpidem) is a short-acting non-benzodiazepine hypnotic indicated for the short-term treatment of insomnia characterized by difficulties with sleep initiation.
Uses & Indications
Dosage
Adults
The recommended dose for adults is 5 mg or 10 mg taken orally immediately before bedtime. The maximum daily dose is 10 mg.
Elderly
For elderly or debilitated patients, the recommended initial dose is 5 mg immediately before bedtime.
Renal_impairment
No specific dose adjustment is generally required for patients with mild to moderate renal impairment, but caution is advised.
How to Take
Take orally immediately before bedtime. Do not take with or immediately after a meal, as this may delay the onset of action. Swallow the tablet whole with water.
Mechanism of Action
Zolpidem selectively binds to the benzodiazepine-1 (omega-1) receptor subtype of the GABA-A receptor complex, enhancing GABAergic transmission and producing sedative, hypnotic, anxiolytic, anticonvulsant, and muscle relaxant effects.
Pharmacokinetics
Onset
Onset of action within 30 minutes.
Excretion
Excreted mainly in the urine (approximately 60%) and feces (approximately 40%) as inactive metabolites.
Half life
Elimination half-life is approximately 2-3 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 0.5-3 hours.
Metabolism
Extensively metabolized in the liver via cytochrome P450 enzymes, primarily CYP3A4, CYP2C9, and CYP2C19.
Side Effects
Contraindications
- Hypersensitivity to zolpidem or any component of the formulation
- Severe hepatic impairment
- Sleep apnea syndrome
- Myasthenia gravis
- Severe respiratory insufficiency
Drug Interactions
Alcohol
Concurrent use with alcohol can increase CNS depression and impair psychomotor performance.
CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir)
May increase zolpidem exposure and enhance its effects. Dose reduction may be necessary.
CYP3A4 Inducers (e.g., rifampicin, St. John's Wort)
May decrease zolpidem exposure and reduce its efficacy.
Other CNS Depressants (e.g., benzodiazepines, opioids, tricyclic antidepressants)
Additive CNS depressant effects.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include impaired consciousness ranging from somnolence to coma, and cardiorespiratory depression. Treatment should be symptomatic and supportive, with immediate gastric lavage if appropriate. Flumazenil may be considered as an antidote but should be administered with caution.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Zolpidem is excreted in breast milk; therefore, it is not recommended for use by nursing mothers.

Lab Monitoring
- No routine laboratory monitoring is typically required. However, liver function tests may be considered in patients with severe hepatic impairment.
Doctor Notes
- Emphasize short-term use only (typically 7-10 days, max 4 weeks) to minimize risk of dependence.
- Counsel patients on potential for complex sleep behaviors and the importance of immediate discontinuation if these occur.
- Caution should be exercised in elderly patients due to increased risk of falls and CNS depression. Consider lower initial dose.
- Advise patients not to consume alcohol or other CNS depressants concurrently.
- Assess for underlying causes of insomnia before initiating zolpidem treatment.
Patient Guidelines
- Take exactly as prescribed by your doctor, usually just before going to bed.
- Do not take more than 10 mg in a 24-hour period.
- Avoid alcohol consumption while taking this medicine.
