Description
Overview of the medicine
Sentrol 10 mg Tablet contains Escitalopram, a selective serotonin reuptake inhibitor (SSRI) used for the treatment of major depressive disorder, generalized anxiety disorder, panic disorder, social anxiety disorder, and obsessive-compulsive disorder.
Uses & Indications
Dosage
Adults
Starting dose is usually 10 mg once daily. May be increased to a maximum of 20 mg once daily after at least one week, based on clinical response.
Elderly
A lower starting dose of 5 mg once daily is recommended. Maximum dose of 10 mg once daily.
Renal_impairment
No dose adjustment is generally required for mild to moderate renal impairment. Caution is advised in severe renal impairment (CrCl < 30 mL/min).
How to Take
Sentrol tablets should be taken orally, once daily, with or without food. It is recommended to take the medicine at the same time each day.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, thereby increasing the concentration of serotonin in the synaptic clefts and enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Initial antidepressant effects may be seen within 1-2 weeks, but full therapeutic effect usually takes 2-4 weeks.
Excretion
Excreted mainly through urine and feces, both as unchanged drug and metabolites.
Half life
Approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with approximately 80% bioavailability. Peak plasma concentrations are reached in about 4-5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, CYP3A4, and CYP2D6 to its active (S-desmethylcitalopram) and inactive metabolites.
Side Effects
Contraindications
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- •Hypersensitivity to escitalopram or any component of the formulation.
- •Concomitant use with pimozide.
- •Congenital long QT syndrome or acquired QT prolongation.
Drug Interactions
MAOIs
Increased risk of serotonin syndrome. Avoid concomitant use.
Alcohol
Avoid concomitant use due to enhanced CNS depressant effects.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, requiring careful monitoring.
Drugs that prolong QT interval (e.g., antiarrhythmics, antipsychotics)
Increased risk of QT prolongation and ventricular arrhythmias.
Other Serotonergic Drugs (e.g., Triptans, Tramadol, other SSRIs, SNRIs, TCAs, St. John's Wort)
Increased risk of serotonin syndrome.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, tremor, nausea, vomiting, somnolence, tachycardia, ECG changes (e.g., QT prolongation), and in rare cases, seizures, rhabdomyolysis, and metabolic acidosis. Treatment is symptomatic and supportive. Maintain an open airway, monitor cardiac and vital signs. Activated charcoal may be considered. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Escitalopram is excreted into human breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug.
