Overview of the medicine
Seroquel XR (quetiapine extended-release) is an atypical antipsychotic used to treat schizophrenia, bipolar disorder (manic and depressive episodes), and as an adjunctive treatment for major depressive disorder.
Uses & Indications
Dosage
Adults
Initial dose 50 mg once daily, adjusted based on response and tolerability, typically ranging from 400 mg to 800 mg once daily for schizophrenia or bipolar mania; 50-300 mg once daily for bipolar depression; 50-300 mg once daily for MDD adjunctive treatment.
Elderly
Initiate with 50 mg once daily, increase in increments of 50 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe, start with 50 mg/day, increase with caution.
How to Take
Take once daily, preferably in the evening, without food (at least 1 hour before a meal). Swallow tablets whole; do not chew, crush, or split.
Mechanism of Action
Quetiapine is an antagonist at multiple neurotransmitter receptors, including serotonin (5-HT2A) and dopamine (D1 and D2) receptors. Its antidepressant action is mediated by its active metabolite norquetiapine, which is a potent norepinephrine reuptake inhibitor (NET) and a partial agonist at 5-HT1A receptors.
Pharmacokinetics
Onset
Clinical effects for acute psychosis may be seen within a few days, but full therapeutic effects may take several weeks.
Excretion
Approximately 73% excreted in urine and 20% in feces.
Half life
Quetiapine: approximately 7 hours; Norquetiapine: approximately 12 hours.
Absorption
Well absorbed after oral administration. Peak plasma concentrations are reached approximately 6 hours after dosing (for ER). Food increases Cmax and AUC by approximately 18% and 15%, respectively.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to form various inactive and one active (norquetiapine) metabolite.
Side Effects
Contraindications
- •Hypersensitivity to quetiapine or any component of the formulation
- •Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin, indinavir, nefazodone, nelfinavir, ritonavir, saquinavir, voriconazole)
Drug Interactions
CNS depressants
Additive sedative effects.
CYP3A4 inducers
Decrease quetiapine levels, requiring dose increase.
CYP3A4 inhibitors
Significant increase in quetiapine levels, contra-indicated.
Drugs that prolong QT interval
Increased risk of cardiac arrhythmias.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, sedation, tachycardia, hypotension, and anticholinergic effects. Treatment is symptomatic and supportive; maintain airway, ensure adequate oxygenation and ventilation, monitor cardiovascular function.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Not recommended during breastfeeding due to potential for adverse effects in the infant.
