Description
Overview of the medicine
Sertraline is an antidepressant of the selective serotonin reuptake inhibitor (SSRI) class. It is used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, social anxiety disorder, post-traumatic stress disorder, and premenstrual dysphoric disorder.
Uses & Indications
Dosage
Adults
For Major Depressive Disorder, start with 50 mg once daily, may increase up to 200 mg once daily. For OCD, start with 50 mg once daily, may increase up to 200 mg once daily. For Panic Disorder, start with 25 mg once daily, then increase to 50 mg once daily after one week, may increase up to 200 mg once daily. 100mg is a common maintenance dose.
Elderly
Similar to adult dosing, but caution and slower titration may be advisable.
Renal_impairment
No dosage adjustment generally required for mild-to-moderate renal impairment. Caution in severe impairment.
How to Take
Administer orally once daily, in the morning or evening, with or without food.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby enhancing serotonergic neurotransmission. It has minimal effects on norepinephrine and dopamine reuptake.
Pharmacokinetics
Onset
Antidepressant effects usually begin within 1-2 weeks, but full therapeutic effect may take up to 4-6 weeks.
Excretion
Primarily excreted in urine (approximately 44%) and feces (approximately 40-45%) as metabolites.
Half life
Approximately 26 hours (22-36 hours). The active metabolite, desmethylsertraline, has a longer half-life (62-104 hours).
Absorption
Well absorbed orally, peak plasma concentrations typically occur 4.5-8.4 hours after administration. Food slightly increases absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, to an inactive N-desmethylated metabolite (desmethylsertraline) and other metabolites.
Side Effects
Contraindications
- Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of discontinuing an MAOI.
- Concomitant use with Pimozide.
- Known hypersensitivity to sertraline or any component of the formulation.
Drug Interactions

Risk of serotonin syndrome.
Pimozide
Increased pimozide levels.
Triptans
Risk of serotonin syndrome.
Warfarin
Potentiation of anticoagulant effect.
NSAIDs/Aspirin
Increased risk of bleeding.

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, vomiting, tachycardia, tremor, agitation, dizziness, and coma. Management is symptomatic and supportive. Maintain an open airway, monitor cardiac and vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts are excreted in breast milk; caution advised during breastfeeding. Discuss with a doctor.
