Overview of the medicine
Setra 100 mg Tablet contains Sertraline, a selective serotonin reuptake inhibitor (SSRI) antidepressant. It is primarily used to treat major depressive disorder, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).
Uses & Indications
Dosage
Adults
Initial dose is usually 50 mg once daily, adjusted in 50 mg increments over weeks. Maximum recommended dose is 200 mg once daily.
Elderly
Similar to adults, but cautious titration may be considered due to potential for decreased clearance and increased sensitivity.
Renal_impairment
No specific dosage adjustment is generally needed for mild to moderate renal impairment. Caution is advised in severe renal impairment.
How to Take
Setra 100 mg Tablet should be taken orally once daily, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Sertraline works by selectively inhibiting the reuptake of serotonin in the brain, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to an enhancement of serotonergic neurotransmission, which helps to improve mood and reduce anxiety.
Pharmacokinetics
Onset
Therapeutic effects may take 2 to 4 weeks to become apparent.
Excretion
Excreted primarily in feces (about 40-45%) and some in urine (about 12-14%) as metabolites.
Half life
Approximately 26 hours for sertraline and approximately 66 hours for its major active metabolite, N-desmethylsertraline.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved 4.5 to 8.4 hours after oral administration.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, CYP2B6, CYP3A4, and CYP2D6 enzymes.
Side Effects
Contraindications
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- Concomitant use with pimozide.
- Hypersensitivity to sertraline or any component of the formulation.
Drug Interactions
MAOIs
Risk of serious, sometimes fatal, serotonin syndrome. Do not use concomitantly.
Pimozide
Increased pimozide levels and cardiotoxicity; contraindicated.
Triptans
Increased risk of serotonin syndrome; use with caution.
Warfarin
May enhance the anticoagulant effect, increasing bleeding risk. Monitor INR.
NSAIDs/Aspirin
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, nausea, vomiting, tachycardia, tremor, agitation, and dizziness. Serotonin syndrome may occur. Treatment is supportive and symptomatic; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C/D (depending on trimester). Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Sertraline is excreted in breast milk; caution is advised when administered to a nursing mother.
