Overview of the medicine
Sevitan-HTZ 20 mg Tablet is a combination medicine used to treat high blood pressure (hypertension). It contains Olmesartan Medoxomil, an Angiotensin II Receptor Blocker (ARB), and Hydrochlorothiazide, a thiazide diuretic. This combination helps to relax blood vessels and remove excess water and sodium from the body, leading to lower blood pressure.
Uses & Indications
Dosage
Adults
The usual recommended dose is one tablet (Olmesartan 20 mg/Hydrochlorothiazide 12.5 mg) once daily, with or without food.
Elderly
No initial dosage adjustment is typically required, but monitoring of renal function is recommended.
Renal_impairment
Use with caution in patients with mild to moderate renal impairment. Contraindicated in severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
Take the tablet orally once daily with or without food. It is recommended to take it at approximately the same time each day.
Mechanism of Action
Olmesartan Medoxomil blocks the effects of angiotensin II, a natural substance that narrows blood vessels, thereby lowering blood pressure. Hydrochlorothiazide is a diuretic that increases the excretion of sodium and water by the kidneys, which also helps to reduce blood pressure.
Pharmacokinetics
Onset
Onset of antihypertensive effect is typically within 1-2 hours for both components.
Excretion
Olmesartan is excreted primarily via bile and urine. Hydrochlorothiazide is excreted unchanged by the kidneys.
Half life
Olmesartan: approximately 13-15 hours. Hydrochlorothiazide: approximately 5-15 hours.
Absorption
Olmesartan Medoxomil is well absorbed from the gastrointestinal tract and rapidly converted to olmesartan. Hydrochlorothiazide is also well absorbed.
Metabolism
Olmesartan is minimally metabolized in the liver. Hydrochlorothiazide is not metabolized.
Side Effects
Contraindications
- •Known hypersensitivity to Olmesartan, Hydrochlorothiazide, or sulfonamide-derived drugs.
- •Anuria (absence of urine production).
- •Severe renal or hepatic impairment.
- •Pregnancy (second and third trimesters).
- •Concomitant use with aliskiren in patients with diabetes mellitus.
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Corticosteroids
May deplete electrolytes, increasing risk of hypokalemia.
Other Antihypertensive agents
Additive hypotensive effects.
Alcohol, Barbiturates, or Narcotics
May potentiate orthostatic hypotension.
Cholestyramine and Colestipol resins
May impair absorption of hydrochlorothiazide.
Antidiabetic drugs (oral agents and insulin)
Dosage adjustment of antidiabetic drugs may be required.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics/Potassium supplements
May lead to hyperkalemia.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension, tachycardia, bradycardia, dizziness, and electrolyte imbalance (e.g., hypokalemia, hyponatremia). Treatment is symptomatic and supportive. If ingestion is recent, gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Contraindicated during the second and third trimesters of pregnancy due to risk of fetal injury or death. Not recommended during lactation as both components are excreted in breast milk.
