Description
Overview of the medicine
Sevoflurane is a non-flammable, volatile halogenated ether used for the induction and maintenance of general anesthesia in adult and pediatric patients for inpatient and outpatient surgery.
Uses & Indications
Dosage
Adults
Induction: 0.5-5% sevoflurane in oxygen/nitrous oxide mixture. Maintenance: 0.5-3% sevoflurane in oxygen/nitrous oxide mixture, adjusted according to patient response and MAC requirements.
Elderly
Lower concentrations may be required; MAC values decrease with age.
Renal_impairment
No specific dose adjustments needed for renal impairment, but caution is advised due to fluoride ion production. Not recommended for severe renal impairment unless benefits outweigh risks.
How to Take
Administered by qualified anesthesiologists or trained personnel using a calibrated vaporizer, ensuring precise concentration delivery via a face mask or endotracheal tube.
Mechanism of Action
Sevoflurane is thought to produce general anesthesia by enhancing the activity of gamma-aminobutyric acid (GABA) at GABA-A receptors, inhibiting N-methyl-D-aspartate (NMDA) receptors, and modulating other ion channels such as potassium channels, leading to neuronal hyperpolarization and reduced excitability.
Pharmacokinetics
Onset
Rapid, typically within minutes for induction of anesthesia.
Excretion
Primarily excreted unchanged via the lungs. Metabolites are excreted renally.
Half life
Rapidly eliminated from the body via the lungs; not typically measured as a classical plasma half-life due to primary pulmonary excretion.
Absorption
Rapidly absorbed via the lungs during inhalation.
Metabolism
Minimal (less than 5%) hepatic metabolism by cytochrome P450 2E1 (CYP2E1) to hexafluoroisopropanol (HFIP) and inorganic fluoride.
Side Effects
Contraindications
- •Known hypersensitivity to sevoflurane or other halogenated ethers.
- •Patients with a known or suspected susceptibility to malignant hyperthermia.
- •History of liver dysfunction, jaundice, or unexplained fever after previous sevoflurane or other halogenated anesthetic administration.
Drug Interactions
Beta-blockers
May increase the risk of bradycardia and hypotension.
Nitrous Oxide
Potentiates the effect of sevoflurane, allowing for lower sevoflurane concentrations to achieve desired anesthetic depth.
Neuromuscular Blockers
Sevoflurane potentiates the action of non-depolarizing neuromuscular blockers, requiring lower doses of these agents.
Opioids (e.g., Fentanyl)
May reduce sevoflurane requirements and can enhance respiratory depression.
Storage
Store at controlled room temperature (20-25°C), protected from light. Keep container tightly closed when not in use.
Overdose
Overdose leads to profound respiratory depression, cardiovascular depression (hypotension, bradycardia), and loss of reflexes. Management involves discontinuing sevoflurane, ensuring a patent airway, providing assisted or controlled ventilation with 100% oxygen, and maintaining cardiovascular function with fluids and vasopressors as needed.
Pregnancy & Lactation
Pregnancy Category B. Should be used during pregnancy only if clearly needed. Caution is advised when administered to a nursing mother, although minimal amounts are expected to be excreted in breast milk.
