Description
Overview of the medicine
Silburn-1-Cream is a topical antibacterial preparation containing Silver Sulfadiazine 1%. It is used to prevent and treat wound infections in patients with severe burns.
Uses & Indications
Dosage
Adults
Apply a layer of cream, approximately 1/16 inch thick, to the affected area once or twice daily. The burn wound should be covered with the cream at all times.
Elderly
No specific dosage adjustment required, but caution with renal impairment.
Renal_impairment
Use with caution due to potential for systemic absorption of sulfadiazine. Monitor serum sulfadiazine levels if extensive application.
How to Take
For external use only. Apply topically to the cleaned and debrided burn wound. A sterile glove may be used for application. Reapply if removed by activity or hydrotherapy.
Mechanism of Action
Silver Sulfadiazine releases silver ions and sulfadiazine, which act synergistically to inhibit bacterial growth. Silver ions bind to bacterial DNA and cell walls, disrupting replication and integrity. Sulfadiazine interferes with bacterial folic acid synthesis.
Pharmacokinetics
Onset
Antibacterial action begins shortly after application.
Excretion
Absorbed sulfadiazine and metabolites are excreted renally.
Half life
Not applicable for topical application to the same extent as systemic drugs, as systemic absorption is minimal.
Absorption
Minimal systemic absorption of silver; sulfadiazine absorption varies but generally low (1-10%).
Metabolism
Absorbed sulfadiazine is metabolized in the liver (acetylation).
Side Effects
Contraindications
- Hypersensitivity to silver sulfadiazine or any component of the cream.
- Premature infants or neonates during the first 2 months of life (due to risk of kernicterus from sulfadiazine absorption).
- Pregnant women nearing term.
Drug Interactions
Enzymatic debriding agents
Silver can inactivate these enzymes.
Topical proteolytic enzymes
Reduced activity when used concurrently.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Do not freeze.
Overdose
Systemic toxicity from topical overdose is rare due to minimal absorption. If significant ingestion occurs, supportive measures and gastric decontamination may be needed. Monitor for sulfonamide-related adverse effects.
Pregnancy & Lactation
Category B for pregnancy. Avoid during late pregnancy and in premature/neonates due to risk of kernicterus. Use with caution during lactation; consult a doctor.

WHO Essential Medicine
YesClinical Trials
Extensively studied and widely used for burn wound infections. Numerous clinical trials support its efficacy and safety in preventing and treating burn wound sepsis.
Lab Monitoring
- Complete Blood Count (CBC) including white blood cell differential, especially if applied to large body surface areas for prolonged periods, due to risk of leukopenia.
- Renal and hepatic function tests if systemic absorption is a concern (e.g., extensive burns, pre-existing organ dysfunction).
Doctor Notes
- Ensure adequate debridement of necrotic tissue before application for optimal efficacy.
- Monitor for signs of hypersensitivity or systemic adverse effects, particularly with large surface area burns.
- Consider alternative treatments if wound infection persists or worsens.
Patient Guidelines
- Clean the affected area thoroughly before applying the cream.
- Apply a thin, even layer of cream. Do not rub vigorously.
- Wash hands before and after applying the cream.
- Avoid contact with eyes. If contact occurs, rinse thoroughly with water.
- Do not use on premature infants or neonates under 2 months of age.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule.
Driving Precautions
Not applicable, as topical cream does not affect driving ability.
Lifestyle Advice
- Maintain good hygiene to prevent further infection.
- Follow wound care instructions provided by your healthcare professional.
