Overview of the medicine
Silcal-D is a combination supplement containing Calcium Carbonate and Vitamin D3, primarily used to treat and prevent calcium deficiency and to maintain healthy bones. Vitamin D3 helps in the absorption of calcium from the gut.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, or as directed by a physician. Tablets can be chewed or swallowed whole.
Elderly
No specific dose adjustment is generally required, but caution with renal impairment is advised.
Renal_impairment
Use with caution; dose adjustment may be necessary. Monitor serum calcium and phosphate levels.
How to Take
Orally, preferably with or after meals to enhance absorption and reduce gastrointestinal upset. It can be chewed or swallowed whole with water.
Mechanism of Action
Calcium carbonate provides an exogenous source of calcium, which is an essential mineral for bone formation and maintenance, nerve function, muscle contraction, and blood clotting. Vitamin D3 (cholecalciferol) is a fat-soluble vitamin that facilitates calcium and phosphate absorption from the small intestine, promotes bone mineralization, and helps regulate serum calcium and phosphate concentrations.
Pharmacokinetics
Onset
Gradual, effects on bone mineral density are long-term.
Excretion
Calcium is primarily excreted via feces and urine. Vitamin D metabolites are excreted mainly in bile and feces, with a small amount in urine.
Half life
Calcium: Variable. Vitamin D3: Metabolites have half-lives ranging from 15-30 days (calcifediol) to 19-48 hours (calcitriol).
Absorption
Calcium absorption varies and is enhanced by Vitamin D3. Vitamin D3 is absorbed from the small intestine.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol).
Side Effects
Contraindications
- •Hypersensitivity to any component of the product.
- •Hypercalcemia (abnormally high levels of calcium in the blood).
- •Hypercalciuria (abnormally high levels of calcium in the urine).
- •Severe renal impairment.
- •Nephrolithiasis (kidney stones) or history of kidney stones.
- •Primary hyperparathyroidism.
Drug Interactions
Corticosteroids
May reduce calcium absorption and impair vitamin D action.
Cardiac glycosides
Hypercalcemia may potentiate the toxic effects of cardiac glycosides.
Thiazide diuretics
May reduce urinary excretion of calcium, increasing the risk of hypercalcemia.
Phenytoin & Barbiturates
May accelerate the metabolism of Vitamin D, reducing its effectiveness.
Tetracyclines & Fluoroquinolones
Calcium can decrease the absorption of these antibiotics. Administer at least 2 hours before or 4-6 hours after calcium.
Bisphosphonates & Thyroid hormones
Calcium may reduce absorption. Separate administration by at least 30 minutes to 4 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia) include nausea, vomiting, constipation, abdominal pain, muscle weakness, polyuria, polydipsia, confusion, and in severe cases, cardiac arrhythmias and renal dysfunction. Treatment involves discontinuing the supplement, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Often recommended during pregnancy and lactation to meet increased calcium and vitamin D needs, but should be taken under medical supervision.
