Description
Overview of the medicine
Silclog 75 mg Tablet contains Clopidogrel, an antiplatelet medication used to prevent blood clots in patients with heart disease, recent stroke, or peripheral arterial disease. It works by preventing platelets from clumping together.
Uses & Indications
Dosage
Adults
Standard dose for MI, stroke, PAD: 75 mg once daily. For ACS (loading dose): 300 mg or 600 mg, followed by 75 mg once daily.
Elderly
No specific dose adjustment generally required, but caution is advised.
Renal_impairment
No dose adjustment necessary for mild to moderate renal impairment. Caution in severe impairment.
How to Take
Administer orally, with or without food. Take at the same time each day.
Mechanism of Action
Clopidogrel is a prodrug that is metabolized to its active form. The active metabolite irreversibly binds to the P2Y12 adenosine diphosphate (ADP) receptor on platelets, preventing ADP-mediated platelet activation and aggregation.
Pharmacokinetics
Onset
Platelet inhibition observed within 2 hours, maximal effect within 3-7 days of repeated dosing.
Excretion
Approximately 50% via urine and 46% via feces within 5 days.
Half life
Active metabolite has a short half-life (approx. 30 minutes), but the effect is irreversible. Parent drug has a half-life of 6 hours.
Absorption
Rapidly absorbed after oral administration, but extensively metabolized (first-pass effect). Bioavailability is low.
Metabolism
Primarily hepatic, via cytochrome P450 enzymes (mainly CYP2C19, CYP1A2, CYP2B6).
Side Effects
Contraindications
- Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage).
- Hypersensitivity to clopidogrel or any component of the product.
- Severe hepatic impairment.
Drug Interactions
CYP2C19 Inhibitors
May reduce clopidogrel's effectiveness.
Proton Pump Inhibitors (PPIs) e.g., Omeprazole
May reduce clopidogrel's antiplatelet effect (CYP2C19 inhibition).
Warfarin, NSAIDs, other antiplatelets (e.g., Aspirin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to prolonged bleeding time and subsequent bleeding complications. Management is symptomatic and supportive; platelet transfusion may be considered if bleeding is severe.
Pregnancy & Lactation
Pregnancy: Use only if clearly needed. Animal studies show no direct harmful effects, but human data are limited. Lactation: It is not known whether clopidogrel is excreted in human milk. A decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother.
