Description
Overview of the medicine
Silmon (Montelukast) is a leukotriene receptor antagonist used for the treatment of asthma and allergic rhinitis, and for the prevention of exercise-induced bronchoconstriction.
Uses & Indications
Dosage
Adults
Adults and adolescents 15 years and older: 10 mg once daily in the evening.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take orally once daily in the evening, with or without food. For asthma, it should be taken in the evening. For allergic rhinitis, the time of administration may be individualized to suit patient needs.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, preventing the actions of leukotrienes (LTC4, LTD4, LTE4). These leukotrienes are potent inflammatory mediators that cause bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment, all contributing to asthma and allergic rhinitis symptoms. By blocking these receptors, Montelukast helps to reduce these effects.
Pharmacokinetics
Onset
Clinical improvement in asthma symptoms is generally seen within one day.
Excretion
Primarily excreted in feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations (Cmax) are reached in approximately 2-4 hours for 10 mg tablets.
Metabolism
Extensively metabolized in the liver primarily by CYP3A4 and CYP2C9.
Side Effects
Contraindications
- Hypersensitivity to montelukast or any component of the formulation.
- Acute asthma attacks (not indicated for rapid relief of acute bronchospasm).
Drug Interactions
Rifampicin
May decrease plasma concentrations of montelukast. Clinical monitoring for asthma control is recommended.
Phenobarbital
May decrease the plasma concentration of montelukast. No dosage adjustment is generally recommended.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, the most frequently reported adverse events were abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive. It is not known whether montelukast is dialyzable by peritoneal or hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Montelukast is excreted in human milk, so caution should be exercised when administered to a nursing woman.
