Description
Overview of the medicine
Silverpac Cream is an antimicrobial cream used to prevent and treat wound infections in patients with second- and third-degree burns. It works by killing bacteria and other microorganisms.
Uses & Indications
Dosage
Adults
Apply a thin layer of cream (approximately 1.6 mm thick) to the affected burn area once or twice daily. The burn wound should be cleaned and debrided before application.
Elderly
Same as adults.
Renal_impairment
Caution advised if extensive absorption is suspected, though usually not an issue for topical application.
How to Take
For external use only. Apply a thin layer to the affected area. Always wash hands before and after application. The cream should be reapplied if it is washed off or removed.
Mechanism of Action
Silver sulfadiazine acts on the bacterial cell wall and membrane, disrupting its integrity and leading to cell death. It also inhibits bacterial DNA replication. The silver ion is slowly released, providing sustained antimicrobial activity against a broad spectrum of bacteria and some fungi.
Pharmacokinetics
Onset
Immediate local antimicrobial action
Excretion
Absorbed silver and sulfadiazine are excreted in urine
Half life
Not clinically significant due to minimal systemic absorption
Absorption
Minimal systemic absorption; approximately 1% of silver and 10% of sulfadiazine may be absorbed from extensive burn areas.
Metabolism
Absorbed sulfadiazine is metabolized in the liver
Side Effects
Contraindications
- •Hypersensitivity to silver sulfadiazine or any component of the cream.
- •Premature infants and neonates less than 2 months of age (due to risk of kernicterus).
- •Pregnancy at term, breastfeeding women (if applied to large areas).
Drug Interactions
Cimetidine
Concomitant use with cimetidine may increase the incidence of leukopenia.
Enzymatic debriding agents
May inactivate enzymatic debriding agents.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Overdose from topical application is unlikely. If excessive amounts are applied, wipe off the excess. In case of accidental ingestion, seek medical attention immediately. Symptoms might include nausea, vomiting, or other signs of sulfadiazine toxicity.
Pregnancy & Lactation
Not recommended in term pregnancy, premature infants, or neonates less than 2 months. Use with caution in pregnant women or those who are breastfeeding, especially if applied to large body surface areas, as systemic absorption may occur.
