Description
Overview of the medicine
Silver Sulfadiazine 1% Cream is a topical antimicrobial agent used for the prevention and treatment of wound sepsis in patients with second- and third-degree burns.
Uses & Indications
Dosage
Adults
Apply a thin layer (approximately 1/16 inch or 1.5 mm thick) to the affected burn area once or twice daily, or as directed by the physician. The burn area should be cleaned and debrided before application.
Elderly
No specific dose adjustment required; use with caution due to potential for increased systemic absorption if extensive application is used.
Renal_impairment
Use with caution. Monitor renal function and blood counts if applied to large body surface areas or for prolonged periods due to potential for systemic absorption of sulfadiazine.
How to Take
For topical dermatological use only. Clean and debride the wound thoroughly. Using a sterile gloved hand, apply a thin, even layer of cream to the affected area. The treated area may be left uncovered or covered with a dressing. Reapply if removed by patient activity or hydrotherapy.
Mechanism of Action
Silver Sulfadiazine works by releasing silver ions and sulfadiazine, which act on the bacterial cell wall and membrane, inhibiting bacterial growth and replication. It has a broad spectrum of activity against many Gram-positive and Gram-negative bacteria, and some fungi.
Pharmacokinetics
Onset
Rapid onset of antimicrobial action upon topical application.
Excretion
Absorbed sulfadiazine and its metabolites are mainly excreted renally.
Half life
Not well-defined for topical use; systemic half-life of absorbed sulfadiazine is approximately 10 hours.
Absorption
Minimal systemic absorption through intact or damaged skin; however, significant absorption can occur when applied to large areas of burn wound.
Metabolism
Absorbed sulfadiazine is primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to silver sulfadiazine, sulfonamides, or any component of the cream.
- Premature infants or neonates (less than 2 months of age) due to the risk of kernicterus.
- Pregnant women near term (third trimester) due to the risk of kernicterus in the fetus.
- Patients with glucose-6-phosphate dehydrogenase deficiency (G6PD deficiency) due to the risk of hemolytic anemia.
Drug Interactions
Cimetidine
May increase the risk of leukopenia (reduction in white blood cells) when co-administered systemically, though relevance for topical application is low.
Enzymatic debriding agents
Silver can inactivate proteolytic enzymes used in enzymatic debridement. Concomitant use is generally not recommended.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Systemic absorption is minimal, but extensive application over large areas or prolonged use could lead to systemic sulfonamide toxicity (e.g., renal impairment, bone marrow suppression, hemolytic anemia). Management includes symptomatic and supportive care, fluid replacement, and monitoring of blood counts and renal function.
Pregnancy & Lactation
Pregnancy Category B. However, contraindicated in pregnant women near term (third trimester) due to the risk of kernicterus in the fetus. Also contraindicated in nursing mothers, as sulfonamides are excreted in breast milk and can cause kernicterus in infants.

YesClinical Trials
Silver Sulfadiazine has been extensively studied in numerous clinical trials for its efficacy and safety in the prevention and treatment of burn wound infections. It remains a standard of care in many burn units worldwide.
Lab Monitoring
- Complete Blood Count (CBC) with differential, especially white blood cell count (WBC), for patients receiving treatment on large body surface areas or for prolonged periods.
- Renal and hepatic function tests if systemic absorption is suspected or for prolonged/extensive use.
Doctor Notes
- Consider monitoring complete blood counts (especially WBC) and renal function for patients with extensive burns or prolonged treatment due to potential systemic absorption.
- Avoid concomitant use with enzymatic debriding agents as silver can inactivate them.
- Ensure patients are screened for G6PD deficiency before initiation, especially in at-risk populations.
- Advise patients on proper wound cleaning and application techniques.
Patient Guidelines
- Apply the cream as directed by your doctor.
- Wash your hands before and after applying the cream.
- Clean the wound as advised by your healthcare provider before application.
- Avoid contact with eyes.
- Report any signs of severe irritation, rash, or changes in blood tests (if monitored) to your doctor.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not apply a double dose.
Driving Precautions
Silver Sulfadiazine 1% Cream is for topical use only and is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
