Description
Overview of the medicine
Simulect is an immunosuppressant medication used to prevent acute organ rejection in adult and pediatric patients receiving a kidney transplant. It is a monoclonal antibody that targets the interleukin-2 receptor on activated T-lymphocytes.
Uses & Indications
Dosage
Adults
20 mg administered intravenously in two doses: first dose within 2 hours prior to transplant surgery, second dose 4 days post-transplant.
Elderly
No specific dose adjustment required for elderly patients.
Renal_impairment
No specific dose adjustment required for patients with renal impairment.
How to Take
Simulect is administered intravenously as a slow infusion over 20 to 30 minutes, preferably through a central venous line. It must be reconstituted and diluted prior to use.
Mechanism of Action
Basiliximab is a chimeric monoclonal antibody (IgG1) that specifically targets and binds to the alpha-subunit (CD25) of the interleukin-2 (IL-2) receptor on activated T-lymphocytes. By blocking the IL-2 receptor, it inhibits IL-2 mediated T-cell activation and proliferation, a key step in the acute organ rejection process.
Pharmacokinetics
Onset
Rapid binding to IL-2 receptors, leading to immunosuppression shortly after administration.
Excretion
Primarily eliminated via protein catabolism; renal excretion is minor.
Half life
Approximately 7.2 days in adults and 4.5 days in pediatric patients.
Absorption
Administered intravenously, resulting in 100% bioavailability.
Metabolism
Catabolized via general protein degradation pathways.
Side Effects
Contraindications
- Hypersensitivity to basiliximab or to any other component of the formulation
Drug Interactions
Live Vaccines
Avoid concomitant administration of live vaccines due to increased risk of infection.
Other Immunosuppressants
Used in combination with cyclosporine and corticosteroids; may enhance overall immunosuppression.
Storage
Store in a refrigerator at 2°C to 8°C. Protect from light. Do not freeze. Reconstituted solution should be used within 24 hours if refrigerated, or 4 hours at room temperature.
Overdose
Limited experience with overdose. In clinical trials, no dose-limiting toxicity was observed. Management should be supportive and directed toward alleviation of symptoms.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnant women only if clearly needed. It is not known whether basiliximab is excreted in human milk; caution should be exercised when administered to a nursing woman.

Clinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of Simulect in preventing acute organ rejection following kidney transplantation when used as part of a standard immunosuppressive regimen.
Lab Monitoring
- Regular monitoring of renal function, liver function, complete blood count (CBC), and signs of infection.
Doctor Notes
- Administer in a facility equipped to manage anaphylaxis and severe hypersensitivity reactions.
- Ensure patient is on appropriate concomitant immunosuppression.
- Monitor for signs of infection and malignancy post-transplant.
Patient Guidelines
- It is crucial to follow your transplant team's instructions exactly.
- Report any signs of infection, fever, or unusual symptoms immediately.
- Do not receive any live vaccines without consulting your doctor.
Missed Dose Advice
As Simulect is administered in a specific timing around transplant surgery, it is critical not to miss a dose. If a dose is missed, inform the transplant team immediately for guidance.
Driving Precautions
Simulect is unlikely to affect the ability to drive or operate machinery. However, due to the underlying medical condition and other concomitant medications, caution should be exercised.
Lifestyle Advice
