Description
Overview of the medicine
Simulect (basiliximab) is a monoclonal antibody used to prevent acute organ rejection in adult and pediatric kidney transplant recipients. It works by blocking the interleukin-2 receptor on activated T-lymphocytes, thereby suppressing the immune response.
Uses & Indications
Dosage
Adults
Total dose of 40 mg, given as two 20 mg doses. First 20 mg dose within 2 hours prior to transplant surgery. Second 20 mg dose 4 days after transplant.
Elderly
No specific dosage adjustment required; use with caution.
Renal_impairment
No dosage adjustment required.
How to Take
For intravenous administration only. Reconstitute with 5 mL of sterile water for injection, then dilute in 50 mL or 100 mL of 0.9% sodium chloride or 5% dextrose solution. Administer by intravenous infusion over 20-30 minutes.
Mechanism of Action
Basiliximab is a chimeric (mouse/human) monoclonal antibody that specifically binds to the alpha-subunit (CD25) of the interleukin-2 (IL-2) receptor on the surface of activated T-lymphocytes. This binding prevents IL-2 from binding to its receptor, thereby blocking IL-2-mediated T-cell activation and proliferation, which are critical steps in the acute immune response to an allograft.
Pharmacokinetics
Onset
Rapid onset; receptor saturation achieved within hours of infusion.
Excretion
Primarily via systemic catabolism, with minimal renal excretion.
Half life
Approximately 7.2 days in adults, 9.4 days in children.
Absorption
Administered intravenously, thus 100% bioavailability.
Metabolism
Not metabolized by cytochrome P450 enzymes. Catabolized into smaller peptides and amino acids like other antibodies.
Side Effects
Contraindications

Other Immunosuppressants
Used in combination, potential for increased immunosuppression.
Storage
Store refrigerated at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light.
Overdose
Limited clinical experience with overdose. Management should be supportive and directed toward alleviation of symptoms.
