Description
Overview of the medicine
Cinnarizine is an antihistamine primarily used to treat various conditions such as vertigo, Meniere's disease, and motion sickness. It works by affecting the vestibular system in the inner ear and also has some vasodilating properties.
Uses & Indications
Dosage
Adults
Vertigo/Meniere's: 15-30 mg two to three times daily. Motion sickness: 25 mg two hours before travel, followed by 15 mg every eight hours during travel if needed.
Elderly
Similar to adults, but caution advised; lower doses may be necessary due to potential for extrapyramidal side effects.
Renal_impairment
Dose reduction may be required.
How to Take
Oral, preferably after meals to minimize gastrointestinal irritation.
Mechanism of Action
Cinnarizine acts as a selective H1-receptor antagonist, blocking histamine receptors and reducing vestibular stimulation. It also has weak calcium channel blocking activity, which helps in improving blood flow, especially in the brain and peripheral vessels. This dual action contributes to its anti-vertigo and anti-motion sickness effects.
Pharmacokinetics
Onset
Effects usually begin within 30 minutes to 1 hour.
Excretion
Approximately 60% excreted via feces and 40% via urine, mainly as metabolites.
Half life
Approximately 4 hours (range 3-6 hours).
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma levels are reached within 1-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6.
Side Effects
Contraindications
- Hypersensitivity to cinnarizine or any component of the formulation.
- Patients with Parkinson's disease, as it may exacerbate symptoms.
Drug Interactions
CNS Depressants
Concomitant use may enhance drowsiness and sedation.
Hypotensive agents
May enhance hypotensive effects.
Anticholinergic drugs
May potentiate anticholinergic side effects.
Storage
Store in a cool (below 30°C) and dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, vomiting, extrapyramidal symptoms, and hypotension. In severe cases, coma and respiratory depression have been reported. Treatment is symptomatic and supportive, including gastric lavage or activated charcoal if appropriate.
Pregnancy & Lactation
Pregnancy Category C. Cinnarizine should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. It is unknown whether cinnarizine is excreted in human milk, so caution should be exercised when administered to a nursing mother. Consult a doctor.
