Description
Overview of the medicine
Sinatide 50 mcg Inhalation Capsule contains Fluticasone Propionate, an inhaled corticosteroid used for the prophylactic management of asthma and maintenance treatment of COPD. It works by reducing inflammation in the airways.
Uses & Indications
Dosage
Adults
Asthma: 50-250 mcg twice daily. COPD: 250 mcg twice daily. Dosage should be individualized and adjusted according to response.
Elderly
Similar to adult dosage; no specific dose adjustment required, but monitor for systemic effects.
Renal_impairment
No specific dose adjustment is required for patients with renal impairment.
How to Take
Administer by oral inhalation using a suitable dry powder inhaler device (e.g., Revolizer, Rotahaler). The capsule should not be swallowed. Follow manufacturer's instructions for using the inhaler.
Mechanism of Action
Fluticasone Propionate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory activity. It reduces inflammation in the airways by inhibiting the release of inflammatory mediators and cytokines, thereby decreasing bronchial hyperresponsiveness and improving lung function.
Pharmacokinetics
Onset
Clinical effects may be seen within 24 hours, but maximum benefit usually occurs after 1 to 2 weeks of regular use.
Excretion
Primarily excreted in the feces as metabolites; approximately 1% is excreted in the urine as metabolites.
Half life
Approximately 7.8-12 hours after intravenous administration; systemic half-life is longer than local effects.
Absorption
Low systemic absorption (typically <1-5%) after inhalation due to high first-pass metabolism in the liver and poor bioavailability.
Metabolism
Extensive first-pass metabolism in the liver by cytochrome P450 3A4 (CYP3A4) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to fluticasone propionate or any component of the formulation.
- Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
Drug Interactions
Ritonavir
Concurrent use may significantly increase fluticasone propionate plasma concentrations, leading to systemic corticosteroid effects (e.g., Cushing's syndrome, adrenal suppression). Co-administration is generally not recommended.
Ketoconazole
Strong CYP3A4 inhibitors like ketoconazole can increase systemic exposure to fluticasone propionate. Use with caution.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely to require any special treatment other than observation. Chronic overdose may lead to systemic corticosteroid effects, including adrenal suppression. Management should be supportive.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if fluticasone is excreted in human milk; use with caution during lactation. Minimal systemic absorption suggests low risk.

Lab Monitoring
- Monitor growth in pediatric patients receiving long-term ICS.
- Monitor bone mineral density in patients at risk of osteoporosis receiving long-term high-dose ICS.
Doctor Notes
- Emphasize proper inhaler technique to optimize drug delivery and minimize systemic absorption.
- Counsel patients on the importance of regular use for prophylactic effect, not for acute relief.
- Advise patients to rinse mouth after each use to reduce the incidence of oral candidiasis.
Patient Guidelines
- Use regularly as prescribed, even when feeling well, for maximum benefit.
- This is a preventive medication and not for immediate relief of acute asthma attacks.
- Rinse your mouth with water and spit it out after each dose to reduce the risk of oral candidiasis (thrush).
Missed Dose Advice
If a dose is missed, take the next scheduled dose at the usual time. Do not take a double dose to compensate for a missed one. Continue with the regular dosing schedule.
Driving Precautions
Sinatide is unlikely to affect the ability to drive or operate machinery.
Lifestyle Advice
- Avoid known asthma/COPD triggers (e.g., smoke, allergens).
- Maintain good oral hygiene.
