Uses & Indications
Dosage
Adults
Schizophrenia: Initial 10-15 mg/day, target 10-15 mg/day. Max 30 mg/day. Bipolar Mania: Initial 15 mg/day. Max 30 mg/day.
Elderly
Lower starting doses may be considered, particularly in those with cardiac conditions or a predisposition to orthostatic hypotension. Dose adjustments should be gradual.

No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Orally, once daily, with or without food. Tablets can be taken whole or dissolved in liquid.
Mechanism of Action
Aripiprazole is thought to work by acting as a partial agonist at dopamine D2 and serotonin 5-HT1A receptors, and an antagonist at serotonin 5-HT2A receptors. It modulates dopaminergic and serotonergic activity in the brain.
Pharmacokinetics
Onset
Antipsychotic effects may take several weeks to manifest fully. Initial response can be seen within days to weeks.
Excretion
Mainly excreted via feces (55%) and urine (25%).
Half life
Approximately 75 hours for aripiprazole and 94 hours for its active metabolite dehydro-aripiprazole.
Absorption
Well absorbed orally, peak plasma concentrations typically reached within 3-5 hours.
Metabolism
Extensively metabolized by cytochrome P450 enzymes, primarily CYP2D6 and CYP3A4, to its active metabolite dehydro-aripiprazole.
Side Effects
Contraindications
- Hypersensitivity to Aripiprazole or any component of the formulation
- Concomitant use with strong CYP3A4 or CYP2D6 inhibitors if not properly dose-adjusted
Drug Interactions
CYP3A4 Inducers (e.g., Carbamazepine, Rifampin)
May decrease Aripiprazole levels; dose increase of Aripiprazole may be necessary.
CNS Depressants (e.g., Alcohol, Benzodiazepines)
May potentiate sedative effects.
CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole)
May increase Aripiprazole levels; dose reduction of Aripiprazole may be necessary.
CYP2D6 Inhibitors (e.g., Quinidine, Fluoxetine, Paroxetine)
May increase Aripiprazole levels; dose reduction of Aripiprazole may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, vomiting, tremor, and tachycardia. Management involves supportive therapy, maintaining an adequate airway, oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered.
