Description
Overview of the medicine
Skimico-2-W Cream is a topical preparation containing Mometasone Furoate, a synthetic corticosteroid with anti-inflammatory, antipruritic, and vasoconstrictive properties, and Miconazole Nitrate, an antifungal agent effective against various dermatophytes and yeasts. It is used for the treatment of various inflammatory dermatoses where a fungal infection is present or suspected.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily, or as directed by a physician. Continue for up to 2-4 weeks or until symptoms resolve.
Elderly
No specific dosage adjustment required; use with caution.
Renal_impairment
No dosage adjustment required due to minimal systemic absorption.
How to Take
For external use only. Apply a thin film of cream to the affected skin area and gently rub it in. Wash hands thoroughly before and after application unless hands are the treated area. Do not cover the treated area with occlusive dressings unless directed by a physician.
Mechanism of Action
Mometasone Furoate acts by inducing phospholipase A2 inhibitory proteins, lipocortins, which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes. Miconazole Nitrate inhibits the biosynthesis of ergosterol in fungi and changes the composition of other lipid components in the membrane, resulting in fungal cell necrosis.
Pharmacokinetics
Onset
Symptomatic relief typically within a few days of initiation.
Excretion
Mometasone Furoate: Primarily excreted in urine and feces. Miconazole Nitrate: Primarily excreted in urine and feces as metabolites.
Half life
Not precisely determined for topical application due to minimal systemic absorption.
Absorption
Mometasone Furoate: Systemic absorption is minimal following topical application (approximately 0.7% of the applied dose). Miconazole Nitrate: Minimal systemic absorption following topical application.
Metabolism
Mometasone Furoate: Metabolized in the liver. Miconazole Nitrate: Metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Mometasone Furoate, Miconazole Nitrate, other corticosteroids, imidazoles, or any component of the cream
- Viral skin infections (e.g., herpes simplex, varicella)
- Tuberculosis of the skin, acne vulgaris, perioral dermatitis, rosacea
Drug Interactions
Warfarin
Topical miconazole may theoretically enhance the anticoagulant effect of warfarin, though systemic absorption is minimal.
Systemic Corticosteroids
May increase the risk of systemic corticosteroid side effects if applied to large areas or for prolonged periods.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause serious systemic effects due to minimal absorption. However, prolonged or excessive use of corticosteroids may lead to local adverse effects such as skin atrophy, striae, or rarely, systemic effects like Cushing's syndrome. In case of accidental ingestion, seek medical advice.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy and lactation only if the potential benefit justifies the potential risk to the fetus or infant. Avoid large areas or prolonged use. Consult a physician before use.
