Description
Overview of the medicine
Slimi 60 mg capsule contains Orlistat, which is used for weight management in overweight and obese adults. It works by reducing the amount of dietary fat absorbed into the body.
Uses & Indications
Dosage
Adults
60 mg capsule three times daily with each main meal containing fat. Can be taken during or up to one hour after the meal. If a meal is missed or contains no fat, the dose can be omitted.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment needed for mild to moderate renal impairment. Data for severe renal impairment is limited; use with caution.
How to Take
Take orally with water, immediately before, during, or up to one hour after each main meal that contains fat.
Mechanism of Action
Orlistat is a reversible inhibitor of gastrointestinal lipases (e.g., gastric and pancreatic lipases). These enzymes are required to break down dietary fat (triglycerides) into smaller absorbable free fatty acids and monoglycerides. By inhibiting these lipases, orlistat prevents the hydrolysis of dietary fats, thereby reducing the absorption of about 25-30% of ingested fat.
Pharmacokinetics
Onset
Fat malabsorption starts within 24-48 hours
Excretion
Mainly excreted via feces (97%), with less than 1% excreted in urine
Half life
Approximately 1-2 hours (elimination half-life is longer, ~13 hours, but systemic absorption is minimal)
Absorption
Minimally absorbed systemically (less than 1%)
Metabolism
Primarily metabolized in the gastrointestinal wall to inactive metabolites (M1 and M3)
Side Effects
Contraindications
- Chronic malabsorption syndrome
- Cholestasis
- Pregnancy and breastfeeding
- Hypersensitivity to orlistat or any component of the formulation
Drug Interactions
Cyclosporine
Orlistat can significantly reduce cyclosporine absorption. Administer cyclosporine at least 3 hours before or after orlistat.
Levothyroxine
Orlistat can reduce levothyroxine absorption. Administer levothyroxine at least 4 hours before or after orlistat.
Warfarin and other oral anticoagulants
Orlistat can reduce vitamin K absorption, potentially affecting anticoagulant effects. Monitor INR closely.
Fat-soluble vitamins (A, D, E, K) and Beta-carotene
Orlistat reduces absorption. Patients should take a multivitamin supplement containing fat-soluble vitamins at least 2 hours before or after orlistat, or at bedtime.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Isolated cases of overdose with orlistat have been reported. No significant adverse events were observed. Management should be supportive and symptomatic.
Pregnancy & Lactation
Contraindicated during pregnancy and breastfeeding due to potential for maternal weight loss and inadequate nutrient absorption impacting fetal/infant growth. Consult a doctor.
