Description
Overview of the medicine
Slipam 30 mg capsule is a benzodiazepine derivative used to treat anxiety disorders, insomnia, and certain other conditions by exerting its anxiolytic, hypnotic, sedative, muscle relaxant, and anticonvulsant effects on the central nervous system.
Uses & Indications
Dosage
Adults
Anxiety: 15-30 mg daily in divided doses or as a single dose at bedtime. Insomnia: 15-30 mg taken at bedtime. Dosage should be individualized.
Elderly
Lower initial doses are recommended, typically 7.5-15 mg daily, to minimize the risk of oversedation and ataxia. Carefully titrate upwards if necessary.
Renal_impairment
Use with caution; dose reduction may be necessary due to potential for accumulation. Monitor closely for adverse effects.
How to Take
Take the capsule orally with a glass of water, with or without food. For insomnia, take it just before bedtime. Do not chew or crush the capsule.
Mechanism of Action
Slipam enhances the effect of gamma-aminobutyric acid (GABA), the main inhibitory neurotransmitter in the central nervous system. It binds to specific benzodiazepine receptors on the GABA-A receptor complex, leading to an increase in chloride ion influx and hyperpolarization of the neuron, which results in decreased neuronal excitability.
Pharmacokinetics
Onset
Anxiolytic/sedative effects typically begin within 30-60 minutes.
Excretion
Excreted primarily in the urine as glucuronide conjugates of the parent drug and its metabolites.
Half life
Parent drug: approximately 20-30 hours; Active metabolites: up to 50-100 hours (for some).
Absorption
Rapidly and well-absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are typically reached within 1-2 hours.
Metabolism
Primarily metabolized in the liver via CYP450 enzymes (e.g., CYP3A4, CYP2C19) to active and inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to Slipam or other benzodiazepines.
- Severe respiratory insufficiency or acute pulmonary insufficiency.
- Sleep apnea syndrome.
- Severe hepatic insufficiency.
- Myasthenia gravis.
Drug Interactions
Antacids
May delay absorption of Slipam, but usually not clinically significant.
CYP3A4 Inhibitors
Drugs that inhibit CYP3A4 (e.g., ketoconazole, fluvoxamine, grapefruit juice) can increase Slipam plasma levels, leading to enhanced effects and increased risk of adverse reactions.
Central Nervous System (CNS) Depressants
Increased sedative effects when co-administered with alcohol, opioids, barbiturates, antidepressants, antihistamines, and other CNS depressants.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms: Drowsiness, confusion, lethargy, ataxia, dysarthria, and diminished reflexes. In severe cases, hypotension, respiratory depression, coma, and rarely death. Management: Supportive care, including maintaining a clear airway and monitoring vital signs. Flumazenil, a benzodiazepine receptor antagonist, may be used in hospitalized patients for reversal of effects, but should be used with caution due to risk of withdrawal seizures.
Pregnancy & Lactation
Pregnancy: Avoid during pregnancy, especially during the first trimester, due to potential risk of congenital malformations. Use in late pregnancy or during labor may result in neonatal depression (e.g., hypotonia, lethargy, hypothermia). Lactation: Excreted in breast milk and may cause sedation, poor feeding, and weight loss in infants. Avoid during breastfeeding.
